As a Senior Occupational Therapist, you will manage a varied and complex caseload, delivering specialist assessments and interventions that promote independence, reablement and hospital avoidance.
Working closely with patients, families, carers and partner agencies, you will develop personalised solutions that help individuals stay safe and well within their own homes and communities.
You will play a key role within the Collaborative Care Team, contributing your specialist occupational therapy expertise to multidisciplinary decision-making and coordinated care planning. Alongside direct clinical work, you will provide advice and support to colleagues, help develop less experienced staff and students, and contribute to service improvements that enhance outcomes for local people.
Interview dates are Tuesday 10th November (Stage 1) and Friday 13th November (Stage 2).
Deliver holistic occupational therapy assessments
for individuals with complex physical, mental health and social care needs,

independence, wellbeing and safety within the home and community.
Develop comprehensive personalised care and support
plans in partnership with patients, families and carers, identifying strengths,
needs, risks and goals that matter most to the individual.
Provide specialist occupational therapy
interventions that promote reablement, self-management, hospital avoidance and
improved quality of life.

Work autonomously to manage a complex caseload,
using advanced clinical reasoning and professional judgement to assess risk and

Provide specialist occupational therapy advice,
guidance and support to colleagues within the multidisciplinary team,
contributing to effective clinical decision-making and person-centred care.
Support the induction, supervision, mentoring and
development of staff, students and peers as required.
Work collaboratively with primary care, community
health services, social care, housing and voluntary sector partners to deliver
coordinated care.
Advocate for patients and support access to
services, resources and community-based support that address wider determinants
of health.
Contribute to service development, quality
improvement, outcome measurement and the reduction of health inequalities,
while maintaining professional standards, clinical governance requirements,
safeguarding responsibilities and accurate clinical documentation.
