Our Unplanned Care Services include:o Rapid Responseo Early Intervention Vehicle (EIV)o Discharge to Assess pathwayso Frailty Hospital@Homeo General Medical Virtual Ward Our Virtual Ward and Hospital@Home services are delivered in close collaboration with colleagues at West Hertfordshire Teaching Hospitals NHS Trust. In this role, you will provide advanced clinical assessment and decision-making primarily within the Rapid Response and Early Intervention Vehicle services, supporting residents to receive safe, effective care at home and enabling timely discharge from acute settings. The post aligns with the four pillars of Advanced Clinical Practice. You will work closely with fellow Advanced Clinical Practitioners as part of a rota, providing robust advanced clinical cover across weekends and bank holidays. This is an excellent opportunity to work at the forefront of innovative and evolving models of care, within a supportive and collaborative team environment. Additional information:o The service operates on a pre-rostered shift pattern, covering 8am-10pm, 7 days per week, including weekends and bank holidayso The role requires travel across the service area; a car driver with access to a vehicle for work purposes is required (unless you have a disability as defined by the Equality Act 2010). Education/Qualification
Essential
- Registered Nurse with current NMC registration or
- Physiotherapist or Paramedic with HCPC registration
- MSc / PGdip Advanced Clinical Practice
- Independent Non-Medical Prescriber
- Evidence of Management/ Leadership training
- Evidence of teaching demonstrated by a suitable qualification or accredited experience
Desirable
- Evidence of continuing professional education and training at master's level.
- Further desirable qualifications in Primary Care/ Accident & Emergency
- Recognised qualification in Minor Injuries
Experience
Essential
- Extensive experience in Community Care and/or Primary
- Care and/ or Accident & Emergency
- Experience in a clinical leadership/ management role within a complex healthcare environment
- Extensive theoretical & practical knowledge relating to assessment & treatment of minor injuries and illnesses
- Proven experience of utilising expert diagnostic and clinical skills to provide autonomous, unscheduled medical care and management of patients presenting with undifferentiated and undiagnosed conditions
- Experience of assessing and providing quality care to people with Long Term Conditions
- Track record of practice excellence
- Proven ability to prioritise and manage own workload with minimal supervision.
- Experience of team / service/ staff management including HR processes
- Experience of implementing evidence-based care and developing others to do so
- Experience of change management and implementing change in practice
- Evidence of innovation in practice
Desirable: - Experience in Community, Urgent community response services
Skills & knowledge
Essential
- Ability to analyse complex problems and to develop practical and workable solutions to address them
- Ability to lead, enthuse, motivate, and involve individuals and the clinical team and have them understand the organisations and your performance expectations
- A strong sense of personal and team accountability coupled to a clear understanding of the boundaries around delegated authority, responsibility, and accountability
- Excellent inter- personal and communication skills
- Ability to communicate effectively both verbally and in writing
- Good working understanding of the changing NHS environment
- Evidence of comprehensive range of clinical skills and expertise relevant to the role
- Clinical knowledge across a range of acute and chronic conditions
- Knowledge of and experience in the provision of care to vulnerable adults and children in line with best practice of safeguarding
