The

role of the post holder will include provision of direct patient care, carrying

out patient consultations and medication counselling, complex medication

reviews, rationalizing and optimizing medicines for mental health conditions as

well as concurrent medicines for those with long term physical health

conditions, prescribing where appropriate and qualified.

There

will also be a role to support other health care professionals in relation to

mental health medicines including responding to queries and education and

training. This role will provide clinical supervision to other members of the

mental health pharmacy team.

Provision of Clinical Pharmacy service to allocated teams

Provision of high quality patient focused pharmaceutical care.

Provison of professional responsibilities of a pharmacist supervising the dispensing of prescriptions at Fieldhead Pharmacy whilst fulfilling the dispensary commitment.

Participation in the Saturday, Bank Holiday and On-call Rotas

In conjunction with the pharmacy team, develop and implement safe and effective policies and procedures in line with New Ways of Working

To provide management to allocated staff including appraisal , development and clinical supervision

To be involved in the development of new services as allocted by pharmacy management team

Develop and participate in Trust-wide medicines management audits and attend allocated governenace groups to represent pharmacy.

KEY RESULT AREAS

  • To be responsible for the clinical pharmacy and prescription monitoring at allocated clinical teams . Be a specialist medication referral point for these teams.

Performance indicators

  • Medicines reconciliation targets for inpatient areas
  • Ensure all service users have access to clinical pharmacy services
  • Screening for all service users admitted to intensive home based treatment services
  • Develop and contribute to patient focused services to allocated teams , including service user groups.
  • Provide appropriate and timely verbal and written information on all aspects of medication usage to service users, carers and healthcare professionals within SWYPFT.
  • Participate in in-house training for pharmacy staff and healthcare professionals , both attending and delivering training.
  • Participate in the training of pre-registration pharmacy graduates and mental health pharmacy diploma students.
  • Participate in the development of the medicines management clinical audit programme in particular the POMH audits and ensure roll out to allocated units.
  • Participate in research and development as required for the development of the clinical pharmacy services.
  • To cover for other senior pharmacists in their absence.
  • If required due to sickness or other absence to work in other localities of the organisation.
  • To carry out CPD in line with the GPhC requirements.
  • To maintain and demonstrate a good clinical pharmacist knowledge.
  • To work within the policies and procedures of the South West Yorkshire Partnership NHS Foundation Trust and also to ensure compliance with the pharmacy policies and procedures from other organisations when working in partnership.
  • To participate in the Trusts Appraisal system.
  • Following qualification as an independent non-medical prescriber, prescribe medication as an independent prescriber within their own sphere of competence and the bounds of their approved prescribing status in accordance with the Trust non-medical prescribing policy.
  • Promote patient choice and understanding when communicating assessment outcomes or prescribing medicines, including provision of high quality information both written and verbal to support this.
  • Be responsible for ensuring the security and safe handling of FP10 prescription forms and to report any loss or theft as per Trust guidelines

Qualifications

Essential

  • Bachelor/Masters degree in Pharmacy.
  • Current registration with GPhC.
  • Membership of CMHP.
  • Post graduate qualification in clinical /mental health pharmacy or equivalent experience.
  • Membership of the Royal Pharmaceutical Society (RPS).

Desirable

  • Clinical or psychiatric diploma or relevant experience.
  • Accredited member of the College of Mental health Pharmacists (CMHP) or RPS Faculty.
  • Postgraduate Certificate and Diploma in Psychiatric Pharmacy.
  • Supplementary prescribing qualification.
  • Management qualification.

  • Provision of medicines information/ advise/ education to service users, carers and healthcare professionals.
  • Evidence of active involvement in audit, service improvement/ change and project work.
  • In the management of staff, services and/ or resources or the equivalent experience.

Desirable

  • Experience of working with persons with mental health problems.
  • Working in other sectors of the pharmacy profession eg primary care, community.
  • Experience of using Pharmacy Dispensary, Patient Record and Medicines Information systems.
  • Supervision of staff.
  • Working with other specialist services such as Learning Disabilities, ADHD, Substance Misuse service.
  • Research and Development.
  • Production of procedures/guidelines.
