The postholder will be responsible for
- Acting as a central point of contact for calls and referrals into service
- Coordinating care and communicating effectively with clinicians, patients, carers, and partner agencies
- Maintaining accurate and timely records using Trust clinical systems
- Supporting multidisciplinary team working to ensure seamless patient journeys
- Escalating concerns appropriately and contributing to service flow and patient safety
- Providing excellent customer service in a fast-paced and pressured environment
- Supporting service audits, data collection, and performance monitoring where required
Essential Criteria
- Excellent communication and interpersonal skills
- Ability to work effectively in a busy, timecritical environment
- Strong organisational and prioritisation skills
- Confident use of IT systems and electronic patient records
- Ability to work independently and as part of a multidisciplinary team
- Understanding of confidentiality, data protection, and safeguarding principles
