You will be responsible to support the delivery of quality health care interventions to patients living in their own environment in the community, in order to meet their health and wellbeing needs, and to facilitate re-ablement support in order to maximise a patients functional independence at point of referral.

You will support individuals and clinical teams to deliver high quality, clinically effective patient care through assessment, planning, implementation and evaluation. Along the way, you'll build strong relationships with a wide range of healthcare professionals and patients.

Training and Qualifications

Essential

  • GCSE Grade A-C in English & Maths or equivalent.
  • NVQ Level 3 in Direct Care or equivalent.
  • Venepuncture Certificate of Competency or willing to work towards.

Experience

Essential

  • Demonstrable experience in a healthcare setting.
  • Ability to work with MS Office package or willingness to work towards.

Knowledge and Skills

Essential

  • Awareness of long-term conditions.
  • Awareness of needs of palliative care patients.
  • Experience in performing basic nursing tasks such as simple dressings.
  • Experience in the administration of insulin and monitoring of blood sugars, or willing to undertake additional training.
  • Able to work independently with guidance.
  • Remains aware of own limitations.
