Come and join our nursing team as a Transition Nurse and play a vital role in supporting children and young people with life-limiting and life-threate...
As a Band 6 Nurse, you will hold a caseload of children and young people and act as a Named Nurse, ensuring the ongoing assessment, planning, implementation and evaluation of care. You will develop strong therapeutic relationships with families, providing continuity, coordination and advocacy whilst ensuring that care remains centred around the individual needs, wishes and aspirations of each child and young person.
A key aspect of this role is supporting young people through transition into adulthood.
You will hold a dedicated caseload of young people in transition, supporting them and their families from approximately age 14 onwards. Acting as a key point of contact throughout the transition journey, you will coordinate person-centred transition planning, helping young people prepare for adult services whilst ensuring their goals, aspirations and changing needs remain at the heart of decision-making.
Working closely with colleagues across Julia's House and with external health, social care, education and adult service providers, you will help develop and embed effective transition pathways that improve experiences and outcomes for young people moving into adult services. You will provide specialist advice and support relating to transition planning, advocacy, the Mental Capacity Act, Best Interests Decision Making, Deprivation of Liberty Safeguards and Liberty Protection Safeguards, helping families navigate what can often be complex systems and services.
To lead and develop transition support for young people and their families across Julias House, ensuring consistent, high-quality practice. To provide specialist advice, education and guidance to colleagues regarding transition, working in partnership with adult health, social care, education and hospice services to support effective transition from childrens services into adulthood.
To ensure transition practice reflects nationally recognised standards and good practice, including NICE guidance, the Mental Capacity Act, Deprivation of Liberty Safeguards and Liberty Protection Safeguards where applicable.
To role model Julia's House values and behaviours at all times.
Julia's House is an award-winning childrens hospice charity dedicated to bringing comfort and care to families across the two counties. Each family we support is unique and so is the care we provide.
