As a Team Leader at Avery Healthcare, located at Acer Court Care Home in Nuthall, Nottinghamshire, you will take on a pivotal role in supporting the home management team. Your core responsibility is to ensure the delivery of high-quality care by creating a safe, caring, innovative, open, supportive, and inclusive environment for residents, visitors, and staff. This role involves leading the person-centred care planning process, which includes implementation, monitoring, monthly evaluations, care plan audits, and developing and executing action plans. Additionally, you will assist in facilitating the smooth transition of residents in and out of the care home, maintaining clear and effective communication with residents' next of kin, when appropriate. Monitoring each resident's well-being is crucial, ensuring that care plans are updated to address any changes or needs that arise. Practicing safe work systems, particularly in handling residents, is a key aspect of this role, ensuring the safety of all involved, including residents, visitors, and staff. Your focus asTeamLeaderwill be tosupportthehome management team in delivering high-quality care with a difference by providing a safe, caring, innovative, open,supportive, andinclusiveenvironment for residents, visitors,and staff.
Other responsibilitieswillinclude
- Leadingin the person-centred care planning processby implementing,monitoring,and undertaking monthly evaluations, completing care plan audits, developing action plans, andbeing responsible forcompletingthe action plans.
- Assisting in the smooth transition of any discharges from the home, including documentation, medication, and keeping the residents' next of kin fully informed, where appropriate.
- Leadingwithmonitoringthewell-being of each resident, being aware of any changeimpactingcare and care planning andensuring that changing needs are met and written records are amended to reflect any variation.
- Practicing safe systems of work across a range of tasks and moving and handling of residents, by assessing risks and having due regard for personal safety and the safety of residents, visitors, and staff.

Our ideal candidatemust
- Have anNVQ 3 or equivalent in care for care homes.
- Two years experience working in a senior position with the resident group.
- Have the ability tocommunicate effectively both verbally and in writing.
- Demonstrate the ability to lead staff and act responsibly in the absence of the manager.

Essential
- The ideal candidate for the Team Leader position must possess an NVQ 3 or equivalent in care for care homes. Additionally, a minimum of two years of experience in a senior position working with the resident group is required. Effective communication skills, both verbal and written, are essential for success in this role. Furthermore, the candidate should demonstrate the ability to lead staff and take on responsibilities in the absence of the manager.
