- Responsible for a complex and diverse caseload, actively working to prevent admission to the acute sector and supporting patients in their preferred place of care in the Out Of Hours 19:00-23:00
- Responsible for maintaining detailed, accurate and confidential patient records, adhering to Trust policies.
- Leading an effective supportive team, working together to provide clinical advice and support to other staff and patients.
- Responsible for the management of a defined caseload of patients, with continuing responsibility for the assessment of care needs, the development, implementation, and evaluation of care programmers.
- Accountable for the standards of care and for determining the activity of your team. This will include acting as a role model for staff, sharing learning and developing practice.
- A key member of the Community Team, providing robust clinical services for patients, maintaining positive communication links other members of the Multi-Disciplinary Team (Day DN-UCR-Westcall GP-Paramedics), providing high quality patient focused care using evidence-based practice.
- Participating in the delivery of highly skilled nursing care to patients, providing End of Life Care, wound and IV-line management and assisting patients to develop self-management skills.
- Your working hours would be mostly 19:00-23:00

- Registered Nurse
- NMC Registration
- Specialist knowledge acquired through post graduate short courses, training or equivalent experience, in a relevant field.

Previous Experience

- Demonstrable experience of working autonomously to deliver high quality and evidence-based nursing care.
- Experience in providing supervision to junior colleagues and students in the completion of routine nursing and clinical skills.
