As part of the Acute Frailty Team, you will provide specialist frailty assessment and management within the Emergency Department (ED) and Same Day Emergency Care (SDEC). The role focuses on delivering rapid Comprehensive Geriatric Assessments, identifying appropriate alternatives to hospital admission, and supporting safe early discharge to improve patient outcomes and reduce unnecessary admissions and readmissions.
You will work collaboratively with multidisciplinary teams, community services, primary care clinicians, patients, and families to coordinate ongoing care and ensure continuity across healthcare settings. Responsibilities also include participating in multidisciplinary meetings, clinical governance, audit, teaching, quality improvement initiatives, and undertaking clinical procedures within your scope of practice.
In addition, you will support the education and development of resident doctors and medical students working on the Frailty ward.

Education and Qualifications
Essential: - Full GMC registration
Desirable: - MD/PhD or equivalent; - MRCP or equivalent; - ALS
Experience & Knowledge

- Wide experience in Older Peoples Medicine and Acute Frailty
- Ability to work collaboratively with allied health care professionals in primary and secondary care
Desirable: - Knowledge of primary and community healthcare systems and IT
Academic Achievements
Essential
- Ability to use research outcomes in clinical practice
- Knowledge of using evidence base medicine to support decision making
- Experience of conducting clinical audit
Desirable: - Involvement in research projects and published papers

- Computer literate
- Ability and enthusiasm for teaching health care professionals and medical students in an Older Peoples Medicine setting
- Capable of organising and prioritising work effectively
