- To assess
design, and deliver specialised interventions, exercise and education
programmes for patients referred to the service.
- To monitor
evaluate and modify treatment in order to measure progress and ensure
effectiveness of treatment plans.
- To apply
advanced level of understanding of the effect of disability and provide
training and advice on lifestyle changes and adaptations to the clients
environment.
- To support the
assessment and review the management of patients/clients under the care of the
GP and nursing team.
- To act as a
specialist clinical resource for the services on the clinical management of
intermediate care patients and develop appropriate training and education
programmes, for staff, patients and carers.
- To maintain
patient records and documentation which conform to Trust and professional
standards, using the specified electronic patient record.
- To design and
implement ways in which service users views can be sought and inform future
service delivery.
- Maximise
opportunities for health promotion/education/self-care during all contacts with
patient/client.

Experience
Essential
- Post qualification experience relevant to this role.
- Experience of working with people with acute or chronic Long Term Condition
- Experience of multi-disciplinary working

- Experience of supervising junior staff and students.

- Evidence of leadership qualities.
- Ability to understand and critically evaluate outcome measures.

- Evidence of building sound effective working relationships across a range of agencies within the health community.
- Knowledge of relevant legislation and current practice within Intermediate Care/ rehabilitation and community services
- Knowledge of current government and local agenda effecting delivery of services
- Effective communication skills
- IT knowledge and skills
- Knowledge of the principles of clinical governance and its applications
Desirable
- Participated in service developments.
- Experience in participating in clinical audit.
Physical Skills

- Car Driver with access to a vehicle for work purposes
Qualifications

- Registered with the HCPC
- Occupational Therapy Degree
- Willingness to undertake a teaching qualification practice educator

- Leadership qualification
