You will work with individuals / families and carers, to identify eligible needs through undertaking Care Act Assessments.You will be supporting individuals to develop independence and wellbeing, through an asset, strength-based approach, utilising community services and groups, and where required commissioning services within agreed procedures and review the effectiveness of these services.

JOB PURPOSE

To provide a

comprehensive case management services that contributes and supports the

promotion of independence, good health and well-being for adults with complex

health and social care needs who are North East Lincolnshire residents

To carry out

assessments and reassessments of need and commission services in accordance

with agreed procedures.

To conduct formal

reviews of individual packages of care.

To manage and

co-ordinate a defined caseload of adults / older people and people with

physical and learning disabilities, responding to need and amending care plans

and packages of care as necessary.

To work in partnership

with service users, families, carers, health colleagues and other agencies in

the assessment, development, delivery and review of care services.

PRINCIPAL

DUTIES & REPONSIBILITES

PRACTICE RESPONSIBILITIES: Health and Social

Carries out a full

range of assessment services to adults and their families and carers, following

organisational procedure in Line with the Care Act 2014 and seeking advice as

necessary.

Identify need/risk and

support the development of integrated health and social care plans with service

users and their families carers Support

the development of individual contingency plans, support networks and self-help

mechanisms, utilising statutory, third sector and an individual assets.

Micro-commission

resources as identified following assessment.

To promote service

users and carers involvement in all aspects of care planning and service

delivery in line with local guidance, whilst respecting the rights,

responsibilities and wishes of the person concerned.

Reports back on

outcomes, quality of service and / or change in circumstances that requires

further assessment by a health or social practitioner.

Promotes the use of

Direct Payments and / or Individual Budgets to support individual control and

independence.

Is familiar with and

adheres to the Safeguarding Adults policies and procedures to ensure protection

of vulnerable people from harm or abuse; including acting as an alerter to any

issues that present whilst carrying out their duties.

Liaises closely with

other agencies, planning, arranging, co-ordinating and monitoring a range of

services to meet the needs of adults and their families and carers to the

required standards, arranging and chairing reviews as necessary.

Maintain electronic

case records using relevant Adult Social Work systems i.e. SystmOne / ContrOcc.

COMMUNICATION

Form strong

partnerships with Local Authority, Primary and Secondary Care, Housing and the

Voluntary Sector to contribute to the case management process and link with

Health Practitioner colleagues.

Maintain and enhance

good professional working relationships with colleagues in the team, service,

other professionals and agencies.

Effectively

communicates with adults who have complex conditions, their families and carers

to establish links and networks for support.

Communicate and discuss

sensitive issues such as personal and emotional care needs and how these needs

may be met with sensitivity and understanding.

To maintain accurate

and up to date written and electronic records using relevant IT packages and

databases.

To effectively

communicate the needs, wishes and requirements in respect of individual care

with colleagues, professionals and other agencies.

SUPERVISION / MANAGEMENT RESPONSIBILITIES

Manage own caseload and

time effectively.

Submit accurate reports

and information as required, ensuring effective case management, outcomes and

quality standards.

Seek out guidance and

support regarding complex issues that may present with their caseload.

To manage conflicting

priorities and work to tight deadlines.

Make decisions within

delegation and know when to seek advice and guidance.

STRATEGIC RESPONSIBILITIES

Focus is an

independent Social Enterprise (Community Interest Company) providing Adult

Social Work across North East Lincolnshire. We believe that being an

independent social work practice provides us with unique opportunities to be

more flexible and creative in our approach. This benefits our service

users, their families and carers and our staff.

We can provide you with a 34 hour

working week (Monday to Friday) to cover between 8-6pm, a generous leave

allocation and support / training to further enhance your development. This is

on a 12 month fixed term contract.

Please note, we are not currently in a position

to provide sponsorship to those individuals that may require it. Therefore,

applicants must have current and unrestricted right to work in the UK.

Knowledge/Understanding

Essential

  • Knowledge of assessment, care planning and reviewing
  • Have a sound understanding of older people or people with a physical/learning disability
  • Knowledge of Safeguarding Adults policies and procedures
  • Have an understanding of individuals rights and choices and the importance of empowerments and promotion of independence

Qualifications

Essential

  • Educated to GCSE Standard (Grade C / Level 4 or above) in English and Maths, or Literacy and Numeracy Award equivalent (Level 2 of the National Qualification Framework)

Skills/Abilities

Essential

  • Excellent verbal communication skills with a high degree of customer focus
  • Ability to maintain a professional approach at all times
  • Ability to assess Service Users and carers and develop care plans to meet identified need
  • Ability to undertake and achieve the 3 year Social Work Degree qualification
  • Ability to write accurately and concisely with a high level of attention to detail.
  • High standard of IT skills
  • Ability to prioritise and management work to timescales, both academically and within a practice-based setting.

Experience

Essential

  • Experience of working and engaging with vulnerable adults, families and carers in either professional, personal or voluntary capacity
