We are looking for a caring and enthusiastic Occupational Therapist to join our Hospice at Home team, supporting patients in the comfort of their own homes. In this role, you’ll take a holistic approach, supporting patients’ physical, psychological, social and cognitive needs. Working both independently and as part of a multidisciplinary team, you will assess, plan and deliver personalised interventions, promoting a rehabilitative and enablement-focused approach to care. You’ll build meaningful relationships with patients, families and carers, using advanced communication skills to support them through challenging and sensitive situations.

Working alongside colleagues and external partners, you’ll help ensure patients are supported to remain in their preferred place of care wherever possible. This is a rewarding opportunity to combine your clinical skills with compassionate care in a supportive and collaborative environment. We are looking for someone who shares our values of Integrity, Compassion, Accountability, Respect and Excellence, and who is committed to delivering high-quality, person-centred care.
