We have an exciting new opportunity for someone to join our practice as a part time Senior Care Navigator, supporting our care navigator team and work...
The purpose of the role is to ensure our team of care navigators provide a friendly, efficient and first-class service for our patients and our local clinical team.
Alongside the Site Co-ordinator the postholder will act as the point of contact for all clinicians and other surgery colleagues with any issues.

General
Care Navigator duties ensuring an excellent patient service

Organising
the day-to-day Care Navigator rota and duties alongside the Site Co-ordinator

Supporting
with training new starters and putting together training plans

Supporting
the Site Co-ordinator in planning and holding Care Navigator target meetings, and
ensuring folders reflect this
-
Supporting
the team with difficult calls
-
Oversee
the smooth running of the reception area so that it is welcoming, friendly and
clean and ensuring up to date information displayed around the practice

Support
Site Co-ordinator with inductions for new staff,locums,trainees,registrars etc

Weekly
room planning
-
Weekly
monitoring and reporting of calls for team members alongside Site Co-ordinator

Order
stationary, toners and hygiene products for clinical rooms

Pick
up on the day maintenance issues alongside Site Co-ordinator

Weekly
testing of fire alarm's alongside Care Navigation team

Monitoring
all stationery levels including printer toners, drums and placing orders.
according to organisations preferred supplier.

Experience
Essential
- Qualifications and experience
- Previous experience of supervising people in a fast paced environment
- A desire to develop the skills and confidence within a team to fulfil their potential whilst continuously improving patient services
- Willingness and ability, to assertively manage difficult conversations with patients
- Ability to build strong working relationships with your team, the wider surgery colleagues and our muti-disciplinary PCN team, and external agencies
- Excellent communication skills both orally and in writing
- Ability to work under pressure while maintaining a positive professional attitude
