Norfolk Primary Care is looking for a compassionate and
motivated Salaried GP to join our Vulnerable Adult Service at Under One Roof in
Norwich.
This is an opportunity to work within a multidisciplinary
team providing high-quality, person-centred primary care to adults experiencing
multiple disadvantage, including homelessness, substance misuse, mental health
conditions and social exclusion.
You will work closely with support workers, nurses and
partner organisations to improve access to healthcare for some of the most
vulnerable members of our community.
This is a fully onsite clinical role based at Under One
Roof, Norwich. Attendance on site is required for all contracted sessions.
- Provide high-quality, patient-centred primary care consultations.
- Undertake face-to-face and telephone consultations.
- Assess, diagnose and manage acute and long-term conditions.
- Prescribe safely and in accordance with local prescribing guidance.
- Review and sign repeat prescriptions.
- Make referrals to secondary care and community services.
- Support patients with complex physical, mental health and social needs.
- Work collaboratively with the multidisciplinary team to deliver coordinated care.
- Maintain accurate and contemporaneous clinical records using SystmOne.
- Maintain professional registration, appraisal, revalidation and mandatory training requirements.

Qualifications
Essential
- Fully qualified General Practitioner with current GMC registration and licence to practise.
- Included on the NHS England GP Performers List.
