Job description

Teqniq is searching for a Reablement Coordinator to work in the public sector. 35 hours per week. 12 week contract. Job Description: Enhanced DBS is required. The post holder will be part of the Reablement Service In-house provider services. Reablement enables individuals to regain or maximise their independence through intensive, short-term support, building on their strengths and enabling people to live healthy independent lives for as long as possible in their own homes, or the place they call home. The Reablement Service is for adults with a range of needs including learning disability, physical and sensory needs, mental health and older people both in community and hospital settings across various locations. The service is currently delivered 365 days from 8am - 10pm. However, the future direction is for the services to become increasingly multi agency and the service may need to change in line with service development requirements. The post holder will be part of a multi-disciplinary team carrying out targeted work to improve the independence of people going through a period of Reablement. They will coordinate a caseload of people receiving a period of Reablement, reviewing progress towards their personalised Reablement goals. They will lead reviews and update Reablement plans and risk assessments, ensuring a person is maximising independence as much as possible during their Reablement journey, and working to meet Reablement goals. The post holder will be responsible for transitioning the person out of the service by 6 weeks or before, completing strengths based reviews and arranging formal long term care only where necessary. The post holder will report to the Reablement Senior Practitioner or Reablement Manager. From April 2022 this role will require you to be fully vaccinated in line with the Government's Coronavirus (COVID-19) Making vaccination a condition of deployment in the health and wider social care sector for CQC regulated services. Flexible working over a 7-day working week, to cover hours 8am-10pm will be required to fit in with the needs of the service:

To ensure that all there is a strengths based approach demonstrated in the delivery of care and support as detailed in the persons Reablement plan.To ensure that the Care Act 2014 is implemented in all aspects of their role.To work with people using our services, carers, families and other professionals to minimise and manage risk whilst enabling the service user to maintain their chosen lifestyle as far as possible.To promptly feedback any progress or changes in a person's wellbeing, physical health or circumstances to the duty manager or emergency services.To keep concise legible records and complete documentation which complies with organisational and professional standards.To be able to use information technology to organise and carry out daily tasks.To actively recognise the need to liaise with other services and health professionals to ensure best support is provided to meet the needs of the service users and their families.To maintain excellent standards of Infection control at all times adhering to universal precautions.To attend and contribute to multi- disciplinary and other meetings as required.To meet contractual agreement by attending regular supervision.To use and assist others in the use of information technology systems to carry out duties in the most efficient and effective manner.To achieve agreed service outcomes and outputs, and personal appraisal targets, as agreed by the line manager.To undertake training and constructively take part in meetings, supervision, seminars and other events designed to improve communication and assist with the effective development of the post and post holder.To ensure that duties are undertaken with due regard and compliance with the Data Protection Act and other legislation.To follow the Pan London Safeguarding Adults procedures to investigate and manage risk where individuals have experienced or are likely to experience significant harm.
