Processing prescription requests from patients, pharmacies,
and other healthcare professionals within the practice SLA.
Preparing repeat prescriptions in line with the practices
procedures and prescribing protocols.
Checking prescription requests against the patients
medication record and identifying discrepancies or unusual requests for review.
Updating patient medication records accurately and ensuring
changes are documented.
Monitoring repeat medication requirements, including
identifying patients who may need a medication review or be eligible for repeat
dispensing.
Accurately code diagnoses, symptoms, procedures,
investigations and clinical information using SNOMED CT and other relevant
coding systems.
Review and code information from patient consultations,
clinical letters, hospital correspondence, test results and discharge
summaries.
Ensure patient records are complete, accurate, up to date
and appropriately coded.
Identify missing or unclear clinical information and liaise
with GPs and other clinicians where clarification is required.
Support the practice with QOF, enhanced services, IIF and
other contractual reporting requirements.
Maintain confidentiality and comply with GDPR, information
governance and practice policies.
Reception support, including booking nursing appointments,
responding to patient enquiries, signposting patients to appropriate healthcare
services, and providing guidance and support with the use of AskMyGP.
The post holder will work as part of the
practice team, responsible for the timely and efficient repeat prescribing
service to patients. The role will also include the accurate and timely
summarising and clinical coding of patient correspondence. The role will
involve reviewing incoming correspondence, identifying relevant clinical
information, taking appropriate action, and ensuring that items requiring
clinical input are promptly forwarded to the appropriate member of the
Clinical Team.
To ensure a timely and efficient repeat
prescribing service to patients. To summarise and forward in an efficient and
effective manner.
The post holder will contribute to
maintaining accurate, up-to-date and high-quality patient records, while
ensuring that all correspondence is processed efficiently, confidentially and
in accordance with practice procedures and NHS requirements.
The
post holder will be responsible for ensuring all correspondence is dealt with
in an efficient and timely manner and within our service level agreements.
Key duties include
Processing

prescription-related queries from patients and explaining administrative
processes clearly.
Communicating
with community pharmacies regarding prescription issues, missing items, or
requests for clarification.
Accurately code diagnoses, symptoms, procedures,
investigations and clinical information using SNOMED CT and other relevant
coding systems.
Review and code information from patient
consultations, clinical letters, hospital correspondence, test results and
discharge summaries.
Ensure patient records are complete, accurate,
up to date and appropriately coded.
Identify missing or unclear clinical information and
liaise with GPs and other clinicians where clarification is required.
Support the practice with QOF, enhanced

Maintain confidentiality and comply with GDPR,

Identify coding discrepancies or
duplicate/inaccurate entries and make appropriate corrections.
Support the practice in achieving high-quality
clinical data and accurate reporting.
Keep up to date with changes to coding
standards, NHS requirements and primary care contractual requirements.
Work closely with GPs, practice nurses,
administrators and the wider practice team to improve the quality of clinical
records.
Contribute to continuous improvement
of clinical coding processes and data quality within the practice.
Correspond with patients and hospital trusts
regarding relevant clinical and administrative matters.
Reception support, including booking nursing
appointments, responding to patient enquiries, signposting patients to
appropriate healthcare services, and providing guidance and support with the
use of AskMyGP.
Take
opportunities (make every contact count MECC) as appropriate to remind
patients to book checks, for example chronic disease annual reviews, childhood
immunisations, cervical screening

Located on the outskirts of Ipswich town centre, Cardinal Medical Practice is the largest medical practice in Suffolk and was formed as a result of a merger between Chesterfield Drive, Deben Road and Norwich Road Surgeries. The practice is vastly dedicated to offering high quality care to a population of approximately 28,500 patients.
The practice has a clinical team of 4 GP Partners, 11 Salaried GP's, 3 GP Registrars, Advanced Nurse Practitioners, Paramedics, Physicians Associates, Practice Nurses, Nurse Associates, Healthcare Assistants, Care Co-ordinators and additional healthcare professionals. The team is supported by Management and a large administrative team.
