As a Band 6 Practice Nurse, you will:

Deliver high-quality nursing care within a primary care setting.

Undertake assessment, treatment, screening and health promotion activities.

Lead and support the management of long-term conditions including diabetes, asthma, COPD, hypertension and cardiovascular disease.

Provide cervical screening services and contraception provision (including emergency contraception).

Deliver childhood, routine and travel immunisation programmes.

Undertake a range of clinical procedures including venepuncture, ECGs, wound care, urinalysis and spirometry.

Empower patients to improve their health through education, prevention and self-management support.

Participate in clinical governance, quality improvement, audit and service development activities.

Support students and junior colleagues within the practice environment.

Education and Training

Essential

  • Registered Nurse Recognised Diploma Qualifications for Long Term Conditions Management (COPD, Asthma, Diabetes) Cytology and Contraception

Desirable

  • Mentor/Teaching qualification
  • Recognised infection control qualification
  • Prescribing qualification
  • Implant/Coil fitting qualification
  • Leadership qualification

Knowledge

Essential

  • Experience of Nurse led clinics.
  • Experience in contacting and discussing test results with patients.
  • Significant experience at Band 5 (or equivalent level) in relevant primary care or community-based environment.
  • Insight and understanding of current NHS issues in nursing and the NHS.
  • Interest and basic knowledge in all long-term conditions with in-depth knowledge in at least one area.
  • Demonstrate an understanding of audit with experience of undertaking regular audits
  • Experience of compiling procedures and clinical guidelines.
  • Clinical skills, included and not limited to:
  • Venepuncture
  • Blood pressure monitoring
  • ECG
  • Urinalysis
  • Swab taking.
  • Cervical screening
  • Routine childhood immunisations
  • Routine Vaccines
  • Travel health
  • Contraceptive injections
  • Peak flow measurement and Spirometry
  • Nebulising
  • Glucometer assessment
  • Wound care
  • Ear syringing
  • Learning Disability reviews and assessment
  • Long term condition diagnosis, assessment and management including diabetes, COPD, Asthma, Hypertension, Chronic Heart Disease, Lipid management, CKD, ACOS, PAD, AF, Heart failure and NDH
  • Insulin initiation and titration
  • Maintain general tidiness and cleanliness of nurses and treatment room
  • Maintenance and cleaning of general equipment including defibrillator and nebulisers.
  • Health Promotion The post holder should also have a keen interest in developing knowledge and skills across the integrated services.
  • The post holder will be expected to undertake safeguarding training appropriate to their role and adhere to policies and procedures relevant to the area they work.
