You will be responsible for the provision of skilled nursing care to our practice population. You will deliver care within the boundaries of your role, focusing on supporting patients to be healthy, monitoring of long term conditions, health education and screening. You will work collaboratively to meet the needs of patients, ensuring the highest standards of care and supporting the delivery of practice policy and procedures.

Chronic disease managements to include but not limited to: asthma, diabetes, COPD, CHD and LD

Cervical cytology

Sexual health and contraception

Routine treatment room procedures including wound care, dressings and phlebotomy

Vaccination and immunisation for adults and children including travel health

In the role of Practice Nurse, you will be responsible for the provision of skilled nursing care to our practice population. We are situated on the outskirts of Middlesbrough town centre in the Linthorpe village area. Our practice population is 8000. Our multidisciplinary team consists of one GP partner, one non-clinical partner, three salaried GPs, two practice nurses, one healthcare assistant, one clinical pharmacist, two [phlebotomists along with an experienced and supportive administrative team.

We are part of a forward-thinking and dynamic Primary Care Network and benefit from a wide group pf ARRS roles including GP, Advanced Clinical Practitioners, Pharmacists, Social Prescribers, Mental Health Practitioners and Physiotherapists, all supported by a robust administrative team.

We are a medical student teaching practice, teaching all years from 1 to 5 at various times throughout the academic year.

We maintain high QOF/LES/DES achievement and we all try to go the 'extra mile' for our patients and our colleagues.

Qualifications

Essential

  • Essential Qualifications & Professional Registration :
  • Registered Nurse (level 1 Adult Nursing) with current NMC registration
  • Evidence of continuing professional development relevant to general practice
  • Recognised training in immunisation and vaccination
  • Recognised training in cervical screening
  • Experience
  • Essential:
  • Experience working as a registered nurse
  • Experience of assessing, planning, implementing and evaluating patient care
  • Experience of managing patients with long-term conditions
  • Experience of health promotion, disease prevention and patient education
  • Experience of working effectively within a multidisciplinary team
  • Knowledge & Skills
  • Essential:
  • Sound knowledge of professional nursing standards, NMC requirements and relevant clinical guidance
  • Good clinical assessment, decision making and care planning skills
  • Ability to recognise and respond appropriately to deteriorating or acutely unwell patients
  • Good understanding of safeguarding, infection prevention & control, confidentiality and information governance
  • Excellent communication and interpersonal skills
  • Ability to provide clear, appropriate and person-centred health education and advice

Desirable

  • Relevant primary care experience
  • Independent prescribing qualification
  • Relevant qualification in chronic disease management
  • Experience working in general practice or another primary care setting
  • Experience undertaking chronic disease reviews including asthma, COPD, diabetes, hypertension and CVD
  • Experience of childhood and adult immunisations programmes
  • Experience of cervical screening and other relevant screening programmes
  • Knowledge of NHS primary care services and relevant local pathways
  • Familiarity with clinical systems used in general practice
  • Understanding of quality improvement, clinical audit and practice based governance
