Providing primary care nursing support, patient interventions and treatment, you will play a key role in the management of patients with long-term conditions across the Practice population.
This includes
- Health screening including asthma reviews, diabetic reviews, cervical screening
- On-going health education, advice and support for patients with long-term conditions
- Use of recall and reminder systems and referral, as appropriate, to national screening programmes
- Delivery of immunisation programmes
- Provision of primary care nursing services within the scope of practice (e.g. smears, injections, dressings, diabetic and asthma reviews)

A minimum of two years post-registration experience as a Practice Nurse providing care to patients with long-term conditions.
- You will undertake a range of nursing
assessments and provide appropriate care / treatment in conjunction with GPs,
other Practice Nurses or independently as appropriate for the need, according
to Practice policies and protocols.

identifying Practice population needs.

Based at Truro Health Park, we look after over 18000 patients in Truro (Cornwall) and the surrounding area.
You will be part of a dedicated team of GPs, paramedic practitioners, nurses, HCAs and physiotherapists.
Qualifications
Essential
- Nursing degree qualification with full registration of NMC.
- Qualification in long-term conditions.
Knowledge
Essential
- Knowledge of needs of patients with long-term conditions.
- Knowledge of health promotion strategies.
- Awareness of clinical governance issues in primary care.
Desirable
- Knowledge of public health issues in the local area and wider health economy.
Experience
Essential
- Experience of working as a Practice Nurse.
- Post registration experience in managing long-term conditions e.g diabetes, preferably at Band 6 or equivalent.
- High level of clinical skills include cervical cytology, family planning/women's health, immunisation and vaccination, wound management.
- Well-developed communication skills in English, both written and verbal.
- Good IT skills.
