The ideal candidate will have experience of working in Primary Care and a special interest in Diabetic Management. They will have qualifications in chronic disease management, cytology and immunisations.
The role of the Practice Nurse in General Practice is to support the Practice Team in providing safe and effective care for patients.
They work alongside the Nursing Team, GPs, Advanced Practitioners and Pharmacists to provide a wide range of care, from taking patient observations, to delivering vaccinations and providing health promotion advice.
They are also involved in the assessment, screening and management of long-term conditions, caring for patients with acute and chronic illnesses and providing health education. Practice Nurses provide person-centred care, taking into account the individual needs and preferences of each patient. They also work to promote health and wellbeing, enabling patients to make informed decisions about their own health and wellbeing.
The Practice Nurse works alongside a team within General Practice and within the Central Halifax Primary Care Network (PCN).
The Practice Nurse is to have the following key responsibilities and attributes, in delivering health services:
- Delivery of high quality, compassionate care whilst undertaking specific clinical and care tasks, with a focus on promoting good health and independence.
- To provide high-quality holistic and person-centred care to individuals
- To provide routine care to patients as required in accordance with clinical based evidence, NICE Guidelines etc
- Work as part of larger nursing team to support colleagues
- Improve safety and quality of care at every opportunity
- Work with Clinical Lead to take responsibility for developing own clinical competence, leadership, and reflective practice skills within the workplace
- Provide support and supervision to other colleagues within the nursing team; Healthcare Assistants, Phlebotomists, Students etc, and those on learning assignments/placements as required.
- Maintain accurate and contemporaneous patient health records ensuring that clinical data is appropriately recorded with SNOMED codes
- To conduct Long Term Condition Reviews- focusing on Diabetes, with additional COPD & Asthma as required
- To provide wound care, dressings and other clinical tasks as required
- Perform and record clinical observations such as blood pressure, temperature, and pulse etc.
- Provide vaccinations including- Flu, COVID & child immunisations and other relevant clinical tasks as required by the Practice and PCN in line with the competencies of the role
- Provide other services such as venipuncture, Cervical Screening, NHS Health Checks
- Undertake additional training where required to provide other relevant clinical tasks as required by the Practice and PCN, in line with the competencies of the role.
- Enhance own performance through continuous professional development, imparting own knowledge and behaviours to meet the needs of the service
- Care for individuals with Dementia, Mental Health conditions, and Learning Disabilities
- Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to personalised care colleagues and local community and voluntary sector services
- Communicate proactively and effectively with all colleagues across the Practice and PCN, attending and contributing to meetings as required
- Work with your line manager to access regular clinical supervision
- Contribute to and embrace the spectrum of clinical governance
- Maintain a clean, tidy, effective working area
- To assist the nursing team with stock ordering and maintaining rotation of stock to minimise waste
In addition to the primary responsibilities, the Practice Nurse may be requested to:
- Be an accountable professional
- Support delivery of QOF, incentive schemes, quality improvement projects, audits and other quality or cost effectiveness initiatives enhancing service delivery and patient care
- Assist with COVID-19 tasks as required
- Undertake any tasks consistent with the level of the post and the scope of the role, ensuring that work is delivered in a timely and effective manner.
- Duties may vary from time to time without changing the general character of the post or the level of responsibility.
- To have a good understanding of practice protocols and policies
- Undertake all mandatory training and induction programmes
- To act as a chaperone
We are a social, exciting and busy Practice with a proud reputation for delivering high quality primary care. We are fully computerised SystmOne Practice, holding a GMS contract consisting of high achieving QOF and currently hold an Intermediate Care contract for GP Support. You will Practice in a purpose built premises, continuing and developing our excellent relationships with local colleagues in our Primary Care Network, Calderdale ICB and external care teams. We are open to exploring new ways of working & innovative management, excited to develop as individuals and as a Practice through Protected Learning and in-house mentoring.

Qualifications
Essential
- Qualified and registered RGN.

Knowledge and skills
Essential
- Understanding of the scope of the role of the Practice Nurse in context of the nursing and the organisation, and how the role may contribute to service development
- Understands and acts in line with NMC professional standards for practice contained within The Code.
- Knowledge of when to seek advice and escalate to the appropriate professional for expert help and advice
- Understand requirements for NMC professional Revalidation
- Understanding of the importance of following procedures and treatment plans
- Ability to take part in reflective practice and clinical supervision activities
- Ability to organise and prioritise own delegated workload
- Ability to deal with non- routine and unpredictable nature of workload and individual patient contact
- Ability to communicate effectively (written, verbal and non verbal communication) with patients/relatives and carers and all members of the multi-disciplinary team.
- Ability to develop effective and appropriate relationships with people, their families, carers and colleagues
- Ability to support, supervise and assess other learners and health care support workers as required within the clinical setting.
