Competency in basic nursing duties are required for the post along with:

Excellent communication and interpersonal skills

Knowledge and understanding of the range of primary care and community services

Specific nursing skills/qualifications relevant to the post are desirable and/or must be willing to work towards:

  • Cervical cytology
  • Wound care
  • Administering routine childhood & travel immunizations and vaccinations
  • Smoking cessation and associated management
  • Chronic disease management (such as Diabetes, CHD, Asthma, COPD etc)
  • Provision of contraceptive advice and associated services
  • Ring pessary insertion and management
  • Adhere to infection control and prevention
  • Health promotion: by advocating social and environmental interventions that are designed to benefit and protect individual peoples health and quality of life by addressing and preventing the root causes of ill health and not just focusing on treatment and cure.
  • Working collaboratively with GPs and the wider team to provide safe, effective patient care.
  • Participating in audits, clinical governance, mandatory training and continued professional development.
  • Adhering to CQC essential standards of quality and safety.
  • Experience of QOF, LCS and Beccor targets

Practice

Nurse Job Description

The post holder is responsible for the delivery of practice

nursing services care to the practice population. The post is on a job-share

basis and supported by 8 GPs within the practice. Care to be delivered within the boundaries of

the role, focusing upon supporting patients to be healthy and with monitoring

of long-term conditions, health prevention and screening activities. The post

holder will work collaboratively with the general practice team to meet the

needs of patients, supporting the delivery of policy and procedures.

Clinical practice

Support and manage health needs of women

presenting for cervical cytology consultations and to target non-attenders

opportunistically.

Assess and care for patients presenting with

uncomplicated wounds; refer to treatment room clinic as appropriate.

Undertake new patient medicals, elderly

screenings and NHS health checks. Deliver opportunistic health promotion and screenings

for infrequent attenders.

Advise, support and, where appropriate,

administer vaccinations for patients travelling abroad

Undertake ECGs and report to GP

Undertake COPD and Asthma reviews; Peak Flow

Readings and refer for spirometry as required.

Venepuncture

Hypertension management

Urinalysis

Implement and participate in vaccination and

immunisation programmes for both adults and children

Chaperoning and assisting patients where

appropriate who are being examined by another clinician

Assisting GPs with minor surgery and coil

fittings

Home visits for house-bound patients; to undertake

screening and monitoring of chronic disease and annual elderly screens.

Maintaining general tidiness and cleanliness of

own room, stocking and rotating items as required.

Support and advise women requesting information

relating to family planning needs and womens health

Assess, plan, develop, implement and evaluate

programmes to promote health and well-being, and prevent adverse effects on

health and well-being.

Identify, and manage as appropriate, treatment

plans for patients at risk of developing a long-term condition

Prioritise health problems and intervene

appropriately to assist the patient in complex, urgent or emergency situations;

patients to be slotted in between appointments if required.

Promote and deliver evidence-based care for

patients presenting with aural conditions

Drug monitoring: ensure height weight and BMI

measurements are up to date and with all blood test requests.

Processing and preparing samples for path lab

collections.

Adhere to infection prevention and control

principles.

Maintain accurate record keeping using EMIS Web

Manage pessary changes

Communication

Collaborate with the secretaries when need to refer if

required, ensure clear understanding and utilisation of referral mechanisms

within the practice

Prioritise own workload and ensure effective time-management

strategies are embedded in own practice.

Management of risk

Manage and assess risk within the areas of responsibility,

ensuring adequate measures are in place to protect staff and patients

Monitor work areas and practices to ensure they are safe and

free from hazards and conform to health, safety and security legislation,

policies, procedures and guidelines

Ensure safe storage, rotation and disposal of vaccines and

drugs is undertaken.

Undertake mandatory and statutory training

Apply infection control measures within the practice

according to local and national guidelines

Apply policies that reduce environmental health risks, are

culturally sensitive and increase access to health care for all

High Lane Medical Centre is situated in a semi-rural

area, 10 mins from Stockport and 30 mins from Manchester. We are a

forward-thinking training practice, supportive of continued education, personal

development and focused on providing high quality patient care to approx. c.7,500

patients. You will be joining an enthusiastic, well led and

innovative practice team comprising: a principal GP,

eight assistant part-time GPs, practice manager, six receptionists, 2

administrators, two secretaries, two practice nurses and a healthcare

assistant. We have 2 small residential

care homes and a large number above average of elderly patients.

Our local federation is Viaduct Care who provide us

with a pharmacy hub team, seasonal respiratory hub clinics, First Contact

Practitioner (physio), Well-being Coordinator, Mental Health practitioner, and

additional care coordinators. We host

other services, in house; minor surgery, Midwifery services, Ultrasound, Audiology,

joint injections and coil and implants.

We were ranked No. 1 surgery in England in 2022.

Our last CQC rating was GOOD in 2020. We were recently ranked joint number 1

practice in Greater Manchester, and 12th In England. We are an

active member of our local Primary Care Network which includes practices from

Marple, Romiley and Hazel Grove.
