Provide nursing care including wound management, vaccinations, phlebotomy and chronic disease management. Conduct health assessments and cervical screenings. Implement treatment plans for acute and chronic conditions in collaboration with GPs and other healthcare professionals.
Provide patient education on health management, disease prevention and lifestyle changes, and participate in public health and community initiatives promoting wellness.
Manage chronic diseases including diabetes, asthma and COPD in line with clinical protocols and registers. Regularly review and monitor patients, adjusting care plans as required.
Administer vaccinations in accordance with national immunisation schedules, ensuring follow-up and accurate records.
Maintain accurate, detailed and confidential patient records in line with GDPR requirements. Use SystmOne effectively to record, manage and retrieve patient information.
Work collaboratively with GPs, Lead Nurses and the wider multidisciplinary team to provide coordinated, high-quality care. Participate in continuous professional development to maintain and update clinical knowledge.
Contribute to clinical audits and quality improvement activities, using patient and staff feedback to enhance practice and outcomes. Maintain high standards of infection prevention and control, including appropriate PPE use and hand hygiene.
DUTIES AND KEY RESPONSIBILITIES
Clinical Responsibilities
Treatment room: Provide general treatment room nursing care such as wound management, vaccinations, phlebotomy, and chronic disease management.
Assessments and Procedures: Conduct health assessments, screenings, and routine procedures such as ECGs, and blood pressure monitoring.
Treatment Planning: Implement treatment plans for a wide range of acute and chronic conditions in collaboration with the wider healthcare professionals.
Health Promotion and Education
Health promotion and prevention: delivery a range of health care services that support health promotion and prevention including sexual health, and participate in public health programs and initiatives to promote wellness and disease prevention within the community.
Patient Education: Deliver educational support to patients on health management, disease prevention, and lifestyle modifications.
Chronic Disease Management
Specialised care: Organise and run specialised clinics relating to long term conditions management such as: asthma, COPD, Diabetes and other relevant nurse-led clinical areas.
Care Protocols: Follow protocols for the management of chronic diseases and chronic disease registers such as diabetes, asthma, COPD, and cardiovascular disease.
Patient Monitoring: Regularly review and monitor patient health status and adjust care plans as necessary.
Immunisation and Vaccination
Vaccination Schedules: Administer vaccines according to national immunisation schedules.
Patient Follow-Up: Ensure proper follow-up and record management for vaccinations and immunisations.
Administrative and Record Keeping
Documentation: Maintain accurate and detailed patient records, ensuring confidentiality and compliance with GDPR.
Data Management: Utilise EMIS to enter patient data and retrieve patient information efficiently.
Team Collaboration
Multi-disciplinary Teamwork: Work closely with GPs, Lead Nurse, and other healthcare professionals to ensure cohesive patient care and service delivery.
Professional Development: Engage in continuous professional development to keep up-to-date with the latest healthcare practices and innovations.
Students and supervision: Facilitate learning experiences of other clinicians, including trainees, provide supervision, and promote shared learning.
Quality Improvement
Clinical Audits: Participate in clinical audits and contribute to quality improvement meetings to enhance care standards.
Feedback Implementation: Act on patient and staff feedback to improve clinical practices and healthcare outcomes.
Infection Control: Oversee and adhere to strict infection control protocols, including the use of Personal Protective Equipment (PPE) and hand hygiene practices. Undertake IPC audits.
Cold chain and drug stock: Safeguard and contribute to effective cold chain compliancy, ensure appropriate systems are in place and adhered to in regard to drug stock safety and rotation.

The Practice recognises that the role of the Practice Nurse is wide and varied is evolving in nature. Therefore, this job description is not intended to be exhaustive and there will be other tasks and duties that the post holder will be expected to perform within what is ordinarily expected of this role.
The Fuller and Forbes Healthcare Group is a primary care provider with practices currently in Leeds, Devon, Burnley and Gateshead. The surgeries are made up of a diversified clinical teams consisting of GP's, GP Registrars, ANPs, ACPs, and a nursing team, with clinical supervision and mentorship by GPs to medical students.
The organisation is patient-driven and has used patient feedback in the past to develop innovative services such as a bespoke smart phone app, patient-centred practice websites, joint working with the third sector and a bespoke e-consultation service.

Experience
Essential
- Essential criteria
- Educational Background:
- Registered Nurse (RN) with active NMC registration.
- Relevant nursing qualification, ideally with a focus on primary care or community nursing.
- Chronic disease qualifications.
- Clinical Skills:
- Proficient in a range of nursing procedures, including immunisations, chronic disease management, wound care, and health screenings.
- Capable of performing diagnostic tests such as ECGs and spirometry accurately and competently.
- Communication Skills:
- Excellent communication and interpersonal skills, essential for educating patients and interacting with other healthcare professionals.
- Ability to convey complex information clearly and sensitively, adapting the message for diverse audiences.
- IT Proficiency:
- Competent in using SystmOne for patient record management, data entry, and retrieval.
- Organisational Abilities:
- Strong organisational skills, with the ability to manage multiple tasks efficiently and prioritize workload effectively.
- Experience in maintaining comprehensive and accurate patient records.
- Teamwork and Collaboration:
- Proven ability to work effectively both independently and as part of a multidisciplinary team.
- Commitment to collaborative practice and continuous professional development.
Desirable
- Additional certifications in areas such as diabetes management, asthma care, COPD and family planning.
- Training in travel health, smoking cessation, or weight management programs.
- Experience:
- Previous experience in a primary care setting, with a strong understanding of public health and preventative care measures.
- Experience in health promotion and patient education, able to lead workshops or group sessions effectively.
- Research and Quality Improvement:
- Involvement in clinical audits or quality improvement projects, demonstrating an ability to identify and implement service enhancements.
- Skills in evidence-based practice and research, able to contribute to practice protocols and guidelines.
- Additional Competencies:
- Adaptability to change, with a flexible approach to new challenges and practice developments.
- Initiative and problem-solving skills, with a proactive approach to improving patient care and practice efficiency.
