Practice Nurse

• Deliver a high standard of clinical care, supporting the provision of core and enhanced nursing services, including chronic disease management, health promotion, immunisations and screening programmes.
• Undertake cervical cytology, childhood and adult immunisations, and travel vaccinations.
• Support the management of patients with long-term conditions, including diabetes, COPD, asthma, heart failure and stroke.
• Provide sexual health and contraception advice, including contraception initiation and follow-up where appropriately trained.

• Provide high-quality nursing care, including assessment, planning, implementation and evaluation of patient care.
• Undertake cervical cytology in accordance with NHS screening guidance.
• Deliver childhood and adult immunisation programmes, including seasonal vaccination campaigns.
• Provide travel health advice, risk assessments and travel vaccinations where appropriately trained.
• Undertake health promotion, lifestyle advice and disease prevention interventions.
• Provide sexual health services, including contraception reviews, initiation and follow-up where appropriately trained.

• Maintain accurate, timely and confidential clinical records using the practice clinical system.
• Follow current evidence-based guidance, clinical protocols and practice policies.
• Ensure medicines and vaccines are administered safely and in accordance with appropriate authorisation, including Patient Group Directions (PGDs) or Patient Specific Directions (PSDs), where applicable.
• Recognise and respond appropriately to clinical deterioration and emergency situations.
• Maintain infection prevention and control standards.
• Recognise safeguarding concerns involving children and vulnerable adults and follow relevant reporting procedures.
• Participate in clinical audits, quality improvement initiatives and service development.
• Identify clinical risks and report incidents, near misses and significant events appropriately.

• Support the development of a positive learning environment within the practice.
• Participate in the supervision and education of student nurses and other team members, where appropriately trained.
• Share knowledge and experience to support the development of the nursing team.
• Attend nursing meetings, practice meetings and relevant multidisciplinary discussions.
• Contribute to the development and implementation of clinical policies, protocols and new services.

• Ensure appointments and clinical activities are managed safely and efficiently.
• Maintain accurate documentation and appropriate clinical coding.
• Support the effective management of clinical stock, vaccines, equipment and medical supplies.
• Ensure clinical equipment is used safely and report any faults or concerns.
• Maintain patient confidentiality and comply with information governance requirements.
