Key responsibilities include
- Leading and supporting the diabetes service within the practice.
- Undertaking diabetes annual reviews and ongoing disease management.
- Supporting achievement and maintenance of high QOF performance.
- Providing evidence-based care, education, and support to patients with diabetes.
- Delivering chronic disease management clinics.
- Providing a full range of treatment room nursing services.
- Supporting health promotion, prevention, and patient self-management.
- Supervising and supporting two Healthcare Assistants (HCAs).
- Working collaboratively with GPs, pharmacists, PCN colleagues, and the wider multidisciplinary team.
- Contributing to service development and continuous quality improvement initiatives.
To be responsible for the implementation of processes for the
effective management of patients with long-term conditions using evidence-based
practice including care for elderly and housebound patients.
Working as a senior member of the practice multidisciplinary team,
the post holder will ensure nursing services are delivered effectively to the
entitled patient population.
The Practice Nurse will be responsible for a number of clinical
areas such as infection
prevention and control, health promotion, chronic
disease management, health promotion, well women and well man clinics, as well
as actively supporting the practice management team in the reviewing and
delivery of clinical policy and procedure.
Ashley Centre Surgery must ensure that the post holder has
access to appropriate clinical supervision and an appropriate named individual
in the organisation to provide general advice and support on a day-to-day
basis.

Experience
Essential
- Registered Nurse with current NMC registration.
- Demonstrable experience in diabetes management.
- Excellent treatment room nursing skills, including:
- Cervical cytology.
- Vaccinations and immunisations.
- Wound care and dressings.
- NHS Health Checks.
- Long-term condition monitoring.
- Health promotion and lifestyle advice.
- Excellent communication and organisational skills.
- Ability to work autonomously and as part of a multidisciplinary team.
Desirable
- Previous experience working within a GP practice or primary care setting.
- Qualification in diabetes care.
- Advanced Nurse Practitioner qualification.
- Independent Prescriber qualification.
- Experience supervising or mentoring clinical staff.
- Experience of QOF-driven long-term condition management.
- Ability to administer travel vaccination
