The successful candidate will play a key role in delivering high-quality patient-centred care within our busy GP practice. The role will include the management of long-term conditions, with a particular focus on diabetes, including assessment, monitoring, review and development of personalised care plans.
The Practice Nurse will undertake chronic disease reviews, therapeutic monitoring, wound care management, health screenings, cervical cytology, NHS health checks, travel health services, and adult and childhood immunisations. They will be responsible for identifying abnormalities, assessing risk factors, promoting healthy lifestyles, supporting patients to manage their conditions effectively, and referring appropriately when required.
Working collaboratively within a multidisciplinary team, the post holder will provide evidence-based care, contribute to improving patient outcomes, and support the ongoing development of clinical services within the practice. Their specialist diabetes knowledge will help ensure patients receive high-quality care and support to manage their condition and achieve the best possible health outcomes.
The Practice Nurse will be responsible for delivering high-quality, evidence-based nursing care to patients within the practice and will work as part of the multidisciplinary team to support the health and wellbeing of our practice population.
Key responsibilities include
- Management of long-term conditions, including assessment, monitoring, review, and development of individualised care plans.
- Providing specialist diabetes care, supporting patients to manage their condition and achieve improved health outcomes.
- Undertaking chronic disease reviews and identifying, assessing and responding to significant abnormalities.
- Carrying out therapeutic monitoring, promoting medication adherence, evaluating treatment effectiveness and recommending changes where appropriate.
- Providing wound care management, including assessment and treatment of routine and complex wounds.
- Conducting NHS Health Checks and other patient health assessments, identifying health risks and supporting patients to make positive lifestyle changes.
- Undertaking health screening activities, including cervical cytology, and providing advice and support relating to women's health, men's health, sexual health and the health needs of older people.
- Completing risk assessments and recognising safeguarding concerns, including domestic abuse, child protection, vulnerable adults, medication-related issues and wider social factors, escalating appropriately when required.
- Delivering travel health services, including travel risk assessments, vaccination administration and patient education in accordance with current guidelines.
- Administering adult and childhood immunisations autonomously and managing more complex immunisation requirements.
- Promoting health education and disease prevention through patient engagement, advice and support.
- Maintaining accurate and contemporaneous clinical records in line with professional and practice standards.
- Working collaboratively with GPs, nursing colleagues, PCN clinicians, pharmacists and the wider multidisciplinary team to provide safe, effective and patient-centred care.
- Participating in clinical audits, quality improvement initiatives and the ongoing development of nursing services within the practice.
The post holder will be expected to work within the NMC Code of Professional Conduct, practice policies and current national guidance, maintaining their professional knowledge and competence through continuing professional development.

Qualifications
Essential
- Registered Nurse with current NMC registration.
- Post-registration qualification in Diabetes Management (Diploma, Degree or equivalent accredited qualification).
- Evidence of continuing professional development relevant to primary care nursing and long-term condition management.
- Cervical Cytology qualification.
- Immunisation and Vaccination training.
Desirable
- Post-registration qualification in Asthma and COPD Management (Diploma, Degree or equivalent accredited qualification).
Experience
Essential
- Experience working in Primary Care
- Clinical knowledge in the following:
- Hypertension
- CHD
- Asthma
- Spirometry
- Wound care/removal of sutures and staples
- ECGs
- Venepuncture
- Travel medicine
