- Work alongside the wider multidisciplinary team (MDT) to contribute to the care of frail adults in the community setting.
- Be the pharmaceutical expert on medicines use within their area of responsibility and provide a special referral point for healthcare professionals working within the UCR patients in own home and Virtual Frailty Ward teams.
- Lead the Pharmacy aspect of the Trust UCR patients in own home and Virtual Frailty Wards programmes ensuring all legal and professional aspects, as well as robust governance processes are met and in place.
- Provide medicines reconciliation and counselling to improve medication usage and patient understanding of own medicines in the community setting (includes appropriate assessment for compliance aids).
- Clinically assess UCR patients in their own home and Virtual Frailty Wards prescription charts to ensure safe and effective prescribing, including identifying significant drug interactions, providing advice for clinical monitoring.

Experience
Essential
- NHS experience is Essential.
- Experience of working within elderly care or a frailty service.
Qualifications
Essential
- Professional Registration with the General Pharmaceutical Council (GPhC).
- Good Pharmacy Honours Degree (BPharm or MPharm).
Travel
Essential
- UK Drivers Licence and access to a vehicle.
