- Lead the development, implementation of the PCN's strategy and business plan ensuring alignment with National & Local objectives and initiatives.
- Develop and deliver programmes that support the PCN population including population health management, digital and data initiatives
- Provide programme leadership across key areas including prevention, personalised care, health inequalities, urgent care, enhanced access, and integrated care delivery.
- Lead the design, mobilisation, and delivery of new services, ensuring services are integrated, sustainable and focused on improving patient outcomes.
- Support the PCN Board in responding to changing NHS policy, neighbourhood health reforms, and integrated care developments.

Based in the new purpose-built Kings Lynn Health Hub, Kings Lynn Primary Care Network (PCN) serves a patient population of 71,500 patients residing across Kings Lynn and the surrounding area within west Norfolk.
The PCN consists of 3 core GP practices Vida Healthcare, St James Medical Practice, Southgates Medical and Surgical Centre & the Woottons Surgery.
The PCN is led by a clinical director, Dr Caroline Delves.
Experience
Essential
- Experience working within primary care within a management role
- Experience of working in a role that requires a high degree of autonomy
- Experience of managing employees
- Experience of financial management
Desirable: - Experience working within a PCN; - Experience of working with a range of local stakeholders and other providers
Qualifications
Essential
- Educated to A-Level
- Management qualification or equivalent experience (5 years at senior level)
- Project management qualification or equivalent experience (5 years)
