PCN Clinical Pharmacist

To improve patients' health outcomes and the efficiency of the primary care network (PCN) pharmacy team by providing direct, accessible and timely medicines expertise as part of a multi-disciplinary team. To pro-actively complete workloads relating to medicines optimisation issues from other clinical staff to improve patient care, safety and outcomes.

The Clinical Pharmacist must be registered with the General Pharmaceutical Council (GPhC) and enrol in, or already be qualified from, an approved 18-month training pathway or equivalent that equips the Clinical Pharmacist to be able to practice and prescribe safely and effectively in a primary care setting, for example, the CPPE Clinical Pharmacist training pathway, followed by the independent prescribing course.

We are committed to building a multi-disciplinary team and want to build on the pharmacy resource already within the PCN. All our ARRS staff are supported by an education lead to ensure we meet the aspirations of both the team member and the surgeries. The pharmacy team are also supported by a lead pharmacist.

Experience

Essential: - Experience of working in primary care; - Experience of working in a GP practice; - Strong organisational and IT skills

Desirable: - Experience with EMIS; - Experience with Docman; - Experience with Accurx

Qualifications

Essential: - Registered with GPhC

Desirable: - Independent Prescribing; - Additional specialty qualifications; - CPPE Primary Care Pathway
