The type of duties involved will be
- Telephone & some face to face consultations and reviews with patients
- Managing repeat prescriptions
- Responding to patient askmygp queries related to medication.
- Long term condition reviews - training will be provided for this if necessary
- Synchronise medication
- Review urgent prescriptions
- Liaise with hospital pharmacists
- Provide regular support and feedback to practice on prescribing action plans.
- Updating patient records
- Ensuring the practice is compliant with CQC medicine-related standards
- Undertaking risk assessments and ensuring compliance with legislation
- Audits

Responsible for supporting
Market Rasen Surgery in delivering effective medicines management within the
practice, identifying areas for improvement, and initiating and managing
change.
Patient Services
Delivery of patient services as determined by practice
policy and local and national guidelines
Repeat prescribing, daily authorisation of repeats
Synchronise medication
Review discharge letters and A&E attendances
Review clinic letters requesting a change in medication
Manage urgent script requests
Complete a face-to-face review with patients who have a
complex medication regime if needed
Liaise with hospital pharmacists
Complete annual medication reviews for patients with
long-term conditions
Complete structured medication reviews
Run CQC reports to ensure safe management of
prescribing, and contribute to QOF-related prescribing indicators and
medicines-optimisation audits
Review nursing/care/residential home patients
medication reviews and de-prescribing, informed by recognised polypharmacy and
de-prescribing tools (e.g. STOPP/START)
Consider 7-day scripts for vulnerable patients
Face-to-face consultation in clinics and as requested
by the doctor both in practice and domiciliary
Market Rasen Surgery is a medium-sized dispensing practice based in the centre of town, just on the edge of the Lincolnshire Wolds and within easy reach of hospitals in Grimsby, Lincoln, Gainsborough and Louth. We serve a largely rural community of approximately 10600 patients over approximately 200 square miles.
The practice is contracted by the ICB to provide general medical services to our patient population. As well as general medical services, we also provide many additional services, including minor surgery such as vasectomies, hernias and skin procedures in our purpose-built theatre.
In addition to the 2 GP Partners and 8 salaried GPs, we have 3 ACPs, highly skilled practice nurses who carry out general nursing duties, also manages the care of patients with conditions such as asthma or other respiratory disease, diabetes and chronic heart disease.
We also employ an older adults nurse who carries out reviews of patients and supports them with their day to day activities.
The nursing team is supplemented by trained HCAs who take blood, monitor warfarin levels and take basic data such as height and weight.
We also have a team of attached nursing staff who look after our patients. These include care co-ordinator, physios, OT & community midwife. we also have regular outreach clinics in paediatrics, diabetes and psychiatry for the elderly.
The clinical team is supported by the dispensary and administrative staff.
