The post holder will be given the autonomy to specialise

in specific health conditions in line with population health analysis and

work with the wider MDT for conditions such as chronic pain.

If you have a specialism, such as chronic pain, chronic back pain or in tackling health inequalities, this will be considered as an advantage, and we would love to hear about what you believe you could personally bring to our PCN.

We are a very friendly GP Practice with over 70 years of history, led by a stable, forward thinking partnership that is ready to embrace the new look primary care. We have 4 sites and over 47000 patients spread across the city of Derby

Experience

Essential

  • Demonstrates understanding of, and conforms to, relevant standards of practice
  • Minimum of 5 years clinical experience.
  • Solid base of specialised experience in relevant areas.
  • Understanding of the implications of national priorities for the team and/or service
  • Understanding of the process for effective resource utilisation

Desirable

  • Service development
  • Previous experience at band 7

Other Requirements

Essential

  • Satisfactory attendance record
  • Assessed as fit for the post by Occupational Health
  • Able to frequently apply intense concentration
  • Able to carry out moderate to intense physical effort throughout the work period
  • Enhanced DBS
  • Satisfactory references
  • Legally entitled to work in UK
  • Car owner/driver

Desirable

  • Member of relevant Special Interest or Clinical Interest Group

Qualifications

Essential

  • Registered with Health Care Professions Council (HCPC)
  • BSc (Hons) Physio or equivalent
  • Able to demonstrate in practice portfolio experience in core physiotherapy and MSK physiotherapy
  • Member of the Chartered Society of Physiotherapy (CSP)

Desirable

  • May hold or be working towards a prescribing qualification
  • Basic level management/leadership training
  • May hold or be working towards an injection qualification
