Principal Duties and
1. Act as a patient advocate and
patient services navigator.
2. Be a familiar point of contact
within the triage team for other practice staff members and
multi-disciplinary teams.
3. Help navigate health and social
care pathways for patient triage requests for urgent on the day appointments.
4. Escalate to Triage Coordinator Team
Leader or Support GP where appropriate.
5. Feedback to line management to
improve triage processes.
6. Monitor availability of resources
at other locations (e.g. UCC) and identify patients who would be appropriate
for referral to other services.
7. Engage with patients to gather
further information for clinical triage where needed and provide appropriate
signposting when advised by clinical staff.
8. Effective communicator focussing on
what you can do for patients rather than what you cant.
Act as a patient advocate and care navigator.
To be a familiar point of contact within the triage team for other departments
Help navigate health and social care pathways for patients who come through triage.
To liaise with other members of the Multidisciplinary Team (MDT)
To attend meetings regarding appointments and feedback on triage systems
To manage a daily administrative list to coordinate patient pathways
Proactively engage with patients to gather further information for clinical triage, where needed and provide appropriate signposting when advised by clinical staff.
Monitor routine requests and liaising with the duty team when there needs further clinical assessment.
Ensuring smooth operation of same-day triage by assessing appointment availability and knowledge of clinical rotas
Use computers and IT systems extensively to carry out day-to-day duties.

Qualifications
Essential
- GCSE English and Maths grade A-C or equivalent
- Minimum 2 years experience in a Primary Care Reception role
- Ability to multitask
- Excellent customer service skills
Desirable
- Knowledge or Emis Web clinical system
