The Paramedic will act as a specialist clinical resource, working autonomously within the community to assess, diagnose, treat, refer, or signpost patients presenting with undifferentiated or undiagnosed conditions related to minor illness or injury. Collaborating closely with the wider multidisciplinary team, the role involves delivering holistic, patient-centred care, supporting self-care and informed decision-making. The post holder will also be required to carry out home visits and must have access to a vehicle, with reasonable travel expenses reimbursed in line with the organisations policy.
Clinical
- Undertake home
visits as assessed and when appropriate
- Assessment and
triage of acute presentations including telephone triage
- Contributing towards
the running a minor illness/ injury walk-in service
- Assisting in the
training of frontline staff
- Make direct
referrals to primary, secondary and social services within locally
- agreed pathways
guidance and protocols
- ECG and ABPM
interpretation
- Assisting with other
ad hoc clinical tasks
- Ensure complete and
accurate documentation of every patient contact
Professional
- Participate in an
annual individual performance review, taking responsibility for maintaining a
record of own personal and professional development
- Undertake
professional development as required by the role to ensure thatup-to-date
evidence-based knowledge and competence in all aspects of the role is maintained
- Monitor the
effectiveness of your clinical practice through quality andassurance strategies
such as the use of audit and peer review
- Participate in
Practice Meetings
- Adhere to practice
policies, procedures and guidelines
- Diagnosing and
treating patients presenting with minor illness
- Triage and treat
patients wishing to see a health care professional, making anynecessary referrals to
other members of the primary health care team
- Patients with
problems needing referral to secondary care should be discussed with the registered GP
before making such referral.
- Ensure clinical
practice is safe and effective and remains within boundaries ofcompetence, and to
acknowledge limitations.
- Advise patients on
general health care and minor ailments, with referral to other members of the primary
and secondary health care team as necessary
- Works from the
surgery and within communities as an autonomous practitioner caring for patients
and families.
- Works as an
autonomous practitioner, in accordance with the Health Professions Council. Ensure that
personal and professional clinical standards are maintained.
- To undertake
assessment for patients within the community and those attending the surgery, using
diagnostic skills and initiation of investigations where appropriate.
- Prescribe/issue
medications as appropriate following policy, patient group directives and local
pathways.
- To formally and
informally impart knowledge and skills to colleagues promoting peer review and best
practice within the work environment.
- To communicate at
all levels within the team ensuring an effective service is delivered.
- To maintain
accurate, contemporaneous healthcare records appropriate to the consultation.
- Ensure
evidenced-based care is delivered at the highest standards ensuring delivery of high
quality patient care.
- Works with local
policies and procedures.
- Enhance own
performance through Continuous Professional Development, imparting own
knowledge and behaviours to meet the needs of the service.
- To achieve and
demonstrate agreed standards of personal and professional development in order
to meet the needs of the service.
- To participate in
the audit process, evaluation and implementing plans and practice change in
order to meet patient need.
- Contribute
positively to the effectiveness and efficiency of the team and work colleagues.
- Pathological specimens
and investigatory procedures
- Undertake the
collection of pathological specimens including intravenous blood samples, swabs etc.
Perform investigatory procedures requested by the GPs

Qualifications
Essential
- HCPC Registration: Must be a registered Paramedic with the Health and Care Professions Council (HCPC).
- Recognised Paramedic Qualification:
- BSc (Hons) in Paramedic Science, or
- DipHE in Paramedic Practice (for those qualified before degree-level entry became standard).
Desirable
- Advanced Clinical Assessment Skills: Demonstrated ability to assess, diagnose, and manage patients with minor illness or injury.
- Experience in a Primary Care, Community, or Urgent Care Setting: Previous experience delivering autonomous care outside of the ambulance service is often desirable.
- Excellent Communication and Interpersonal Skills: Ability to work collaboratively within a multidisciplinary team and communicate effectively with patients.
- Full UK Driving Licence: Required for home visits and community work.
- Evidence of Continuing Professional Development (CPD): Up-to-date CPD portfolio relevant to scope of practice.
