Palliative Care Occupational Therapist

The post holder will contribute to the provision of specialist palliative care, supporting patient choice in maintaining dignified end of life care, to the people of Cardiff with malignant or non-malignant conditions who have a limited life expectancy. The post holder will be working autonomously and as part of a team. They will deliver therapeutic interventions in patients' homes, and/or when they attend City Hospice as an out-patient or attend our well-being services. They will provide advice, education and support to patients, their carers and other health care professionals. They will also promote advanced care planning, management of complex end of life symptoms and patient and carer expectations , whilst meeting the integrated care needs of the patient.

The post holder will also need to be passionate about delivering quality care to people receiving palliative or end of life care.

Qualifications

Essential: - Degree/diploma in Occupational Therapy; - Professional registration as recognised HCPC; - Fieldwork Education training course

Desirable: - Evidence of recent study (especially that related to palliative care)

Experience

Desirable: - Evidence of experience in palliative care; - Evidence of community working

Skills

Desirable: - Presentation skills

Special knowledge

Essential: - Knowledge of services that link across the primary/secondary care interface to provide integrated care in specialism

Desirable: - Member of special interest group
