As Pall24 Lead, you will provide strategic and operational leadership to the Pall24 team, working closely with the Director of Nursing and Clinical Services, Medical Director, and multidisciplinary colleagues to deliver a safe, high-quality, evidence-based service. You will play a pivotal role in developing integrated care pathways, strengthening partnerships across health and social care, and driving continuous improvement to meet the changing needs of our community.
The successful candidate will have the opportunity to make a tangible impact on patient care and staff experience and service performance while developing their own leadership skills with a supportive senior leadership team.
As part of the requirements of the NWL ICB model of care, this service will provide support for patients and families 24 hours a day. Therefore, the post holder may be expected to work shift patterns including night shifts as part of the service development.
- Job Purpose and scope
Provide clinical leadership and management support to thePall 24 team, ensuring the development and delivery of a responsive, evidence-based,high qualityend of life care service.
Work with lead nurses and other senior staff to promote an integrated and cohesive team aimed at improving care and patient outcomes.
Responsible and accountable for ensuring a safe,effectiveand transparent service; including aspects of Health & Safety, Infection prevention and control, risk management and creating a culture of open discussion.
Work with the Director ofNursing and ClinicalServices and the Medical Director to shape our vision for the future ofthe Pall 24 service, engaging with patients, carers, key local stakeholders in health and social care to improve patient care, promote the role of the hospice, and ensurethe Pall 24 serviceremains central to the delivery of good end of life care.
Contribute to the wider strategic development of the hospice and help to translate local and national strategy relating to community services, enabling us to implement and deliver an effective service that reflects and is responsive to our current and future needs.
Deputisefor the Director ofNursing and ClinicalServices on specific areas of clinical practice and strategic development relating to your leadership remit. In the absence of the Director ofNursing and ClinicalServices,work with leadership peers to respond to enquiries, manage servicesand support the staff asrequired.
Working with the other clinical services leadspromotingan integrated patient service wide culture, which encourages seamless and cross department working and cover for team leads colleagues in their absence.
Participate in the clinical on-callrota.

St Lukes Hospice Caring for Harrow and Brent St Lukes Hospice cares for people approaching the end of their life in Harrow and Brent. We aim to give the people we care for the best possible quality of life, each and every day. Supporting local people and their families is at the heart of what we do focusing on what matters the most to them is at the centre of our care.
The care we give is free of charge, available to all, and respects the cultures, religions, and beliefs in our richly diverse community. Our care is given with compassion and dignity and involves not just looking after the patient, but also includes supporting the people closest to them.
We receive around 40% of our funding from the NHS and the rest is raised from the amazing generosity of the organisations and people who make our care possible. Our local community is a vital partner in our care, kindly supporting us through their donations and fundraising alongside the dedication of over 800 incredible volunteers who freely give us their time to support the best possible end-of-life care for local people.
Qualifications
Essential
- RegisteredAdultnurse
- Post registration qualification in palliative care, oncology or other relevant area of practice
- Educated to first degree levelin relevant area of practice
- Advanced communication skills qualification or willingness to undertake
- Current NMC registration
Desirable
- Qualification or evidence of additional training in leadership
- Study at Masters level or willingness to work towards
- Community nursing experience
- Experience of telephone triage/call centre environment
Skills and experience
Essential
- Experience and skills in Quality Improvement methodology
- Knowledge and understanding of hospice systems and processes
- Knowledge of clinical governance frameworks
- Knowledge of relevant legislation and guidance frameworks
- Expert knowledge of the end of life pathway and the patient-centred approach
Experience
Essential
- Experience in specialist palliative care, oncology or relevant field of clinical practice, commensurate with the seniority of the role
- Have a range of post registration clinical experience at senior clinical level
- Experience of leading and managing a team or service
- Experience of leading a team through change
- Experience of managing and developing people
- Understanding and completing of data recording and reporting
- Experience of auditing and monitoring the quality of care
- Evidence ofrecruitment and workforce development

- Experience of working with and supporting volunteers
- Experience of delegated budget management
