Nurse or Paramedic Urgent Community Response and Hospital at Home

• Provide day-to-day management of the service on a rotational basis in the absence of the Band 8a Clinical Lead.
• Take responsibility for designated areas of clinical practice and operational delivery.
• Prioritise and manage own workload, seeking guidance and support when required.
• To provide skilled, effective and evidence-based nursing care to patients in a variety of community settings.
• To assess referrals made to service, prioritise and respond according to priority, as appropriate both in terms of level and urgency. To respond within agreed timescales as appropriate.
• Utilise advanced clinical and diagnostic skills and expert knowledge to carry out thorough assessments of physical and psycho-social needs in order to develop a comprehensive short term care plan aiming to optimise health and wellbeing that will prevent a hospital attendance / admission.
• Provide direct advanced, autonomous clinical care to a range of patients presenting with differentiated diagnosis or RED flags, undiagnosed and/ or complex health needs and long-term conditions.
• Work within Clinical Guidelines, Policies, Procedures and Patient Group Directions (PGD's) utilising expert judgment to work outside these frameworks where necessary.

Essential

• Registered Nurse or Paramedic with current NMC or HCPC registration
• Evidence of recent and continuing post-registration professional education and training

Desirable

• Experience of working in Primary Care/ Accident & Emergency.
• Experience of assessing and providing quality care to people with Long Term Conditions
• Qualifications in Community and/ or Accident and Emergency Nursing.
• Qualification in teaching or practice-based learning.
• Physical Clinical Assessment Skills Course or willingness to undertake the course.
• Nurse prescribing qualification

Essential

• Evidence of recent substantial post-registration nursing experience
• Proven experience of utilising expert diagnostic and clinical skills to provide autonomous, unscheduled medical care and management of patients presenting with undifferentiated and undiagnosed conditions.
• Experience of assessing and providing quality care to people with Long Term Conditions
• Extensive range of clinical skills and experience including Vene-puncture, cannulation, catheter and bowel care, EOL
• Experience of working within a multi-disciplinary team
• Experience of working with people with complex health needs
• Experience of supervising junior staff and co-ordinating daily activities on a regular basis
• Experience of undertaking comprehensive health needs assessment and writing care plans

• Has high level of competence in a range of clinical skills relevant to the role and patient group
• Competent in physical examination of: ENT, eye, gastrointestinal, respiratory, cardiovascular, neurological, skin, musculoskeletal systems and wound management.
