The post will require the completion of detailed health needs assessments and reviews in line with current legislation and local policy regarding Continuing Health Care. There will be extensive liaison with clients, families, care providers and other professionals within the health and social care sectors. The post holder will also be required to provide case management for complex Continuing Health Care patients and their packages of care, making the role varied from day to day.
The successful candidates will need to be able to demonstrate the ability to complete comprehensive nursing assessments, have good written and verbal communication skills, must have the ability to work in stressful situations and be able to prioritise and organise their own workloads to meet deadlines.
The post holder will be responsible for assessing and determining eligibility for people with complex health needs whom are eligible for NHS funding for long term care and treatments under The National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care, The Mental Health Act. The service also commissions packages of care for clients requiring Transforming Care and Neurological Rehabilitation pathways.
The post holder will be responsible to clinically plan and commission the care required, ensuring that the care is safe, effective and good quality. Communication and relationships with individuals and independent providers is a key part of this role as well as abilities to manage risks, problem solve influence and negotiate to ensure appropriate clinical commissioning case management of complex individuals.
As a registered nurse with experience and interest in community nursing or managing patients with complex needs. You will have experience and skills in case managing patients and carrying out assessments in various care environments.

Essential
- NMC Registered Nurse Part 1 or 2.
Qualifications
Essential
- Registered nurse, Level 1 or equivalent.
- Proven experience in adult nursing assessments.
Desirable
- Knowledge of Continuing Health Care, Funded Nursing Care and the issues surrounding it.
- District Nurse Degree (Specialist Nurse Practitioner).
Skills, Knowledge & Experience
Essential
- Experience of working within a multidisciplinary environment, and with health care professionals.
- Broad range of clinical experience including case management.
- Exceptional accurate documentation and report writing skills
Desirable
- Experience in working within a multidisciplinary decision panel.
- Experience of managing & coordinating others.
- An understanding of Personal Health Budgets (PHBs).
