Our team provide medication review services to patients that maximise cost-effective prescribing and improve the quality of patient care andsupport the achievement of the practices prescribing Quality and Outcomes Framework targets and to be involved with any DES/LES/NHSE/ICB prescribing initiatives and targets.
Our team deal with medication queries including acute medication requests, patient medication and prescription queries.
They liaise with secondary and primary care colleagues to ensure correct medicines are continued following the transfer of care and upon discharge from hospital to ensure full medicines reconciliation to include other transfers of care (e.g. new patients, care home residents), to ensure that medication changes are safely implemented in a timely fashion.
They link with hospital and community pharmacy colleagues to address discrepancies, supply issues and queries within competency and will support patients to manage their medications, for example; synchronisation of medications, reviewing patients suitability for monitored dosage systems, and setting up electronic repeat dispensing.
They ensure actions are undertaken from any Central Alert Notifications regarding medications by running searches for patients on said drugs, agreeing to actions with GPs, contacting patients affected regarding medication changes and logging actions on practice-based evidence spreadsheets.

Poole North Primary Care Network is a supportive and forward thinking organisation which has grown over the last six years to include pharmacists, pharmacy technicians, care coordinators, social prescribers, MSK Practitioner, counsellors other allied healthcare professionals.
Our pharmacy team are co-located at our hub at Birchwood Medical Centre where the technicians are supported every day by an on duty Pharmacist.
The team is supportive and friendly and new recruits are guided through their first few months with support and regular supervision.
To undertake this role you need to have either done or be willing to undertake the PCPEP course and be a fully qualified and registered Pharmacy Technician.
