Role: Qualified Social Worker
Service: Adult Social Care (Mental Health & Reablement)
Location: Leicestershire County Council (Hybrid Working)
Pay Rate: £34.00 per hour
Contract: Full-Time, Ongoing Interim Contract
Leicestershire County Council is seeking a skilled, empathetic Social Worker to join their specialized Mental Health & Reablement Team. This service bridges the gap between acute mental health recovery, community integration, and short-term reablement, helping individuals regain their autonomy and build sustainable independence within their local communities. Core Responsibilities Operating as part of an integrated, multidisciplinary service, your daily duties will include:
Strengths-Based Assessments: Conducting comprehensive social care assessments under the Care Act 2014, prioritizing reablement goals and person-centered support plans.
Mental Health Recovery Support: Managing a varied caseload of adults presenting with complex mental health needs, supporting their transition from hospital or secondary mental health care into community settings.
Mental Capacity & Safeguarding: Completing Mental Capacity Assessments (MCA), Best Interest decisions, and managing adult safeguarding enquiries to protect vulnerable individuals.
Multidisciplinary Collaboration: Working closely with health professionals, occupational therapists, community mental health teams (CMHTs), and secondary care services to deliver seamless, reablement-focused care pathways.
Mandatory Candidate Criteria
Professional Registration: Must be a Qualified Social Worker with an active, valid Social Work England (SWE) registration.
Adult Services Experience: Substantive post-qualifying experience within statutory Adult Social Care, Adult Mental Health, or Reablement/Intermediate Care teams.
Legislative Knowledge: Strong, applied working knowledge of the Care Act 2014, Mental Health Act 1983/2007, and the Mental Capacity Act 2005.
Communication & Resilience: Exceptional interpersonal skills with the ability to build rapport quickly with individuals, carers, and multi-agency partners.
