The role of Medical Lead at HCRG Care Group offers the opportunity to make a significant impact on the care of older people across both inpatient and community services. The candidate will provide medical oversight and care, focusing on older adults with frailty, multimorbidity, and advanced clinical needs. The Medical Lead will collaborate with multidisciplinary teams to deliver person-centred care, drive excellence in patient outcomes, support medical colleagues, and contribute to strategic service development. The position is based primarily at Swindon Intermediate Care Centre and Savernake Hospital, Wiltshire. Key responsibilities include providing clinical leadership, enhancing patient pathways, maintaining clinical accountability, and leading continuous improvement through audits and initiatives.

Qualifications

Essential

  • Full GMC registration with a current licence to practise. Significant post-registration clinical experience, including the assessment and management of patients in community and/or inpatient settings. Strong experience caring for older adults, frail patients, and those with long-term or complex health conditions. A proven ability to work effectively within multidisciplinary teams. Commitment to continuing professional development (CPD) and maintaining up-to-date clinical knowledge.
