Be a single point of contact for patients, carers and the healthcare team - triaging sensitive enquiries and referring to the appropriate department within the hospital. Using own judgement as to what requires immediate action.
Communicate with relevant departments and organisations to ensure that the patient's pathway is as timely as possible and all delays are avoided.
Communicate with key departments to ensure all tests and investigations are booked promptly and any issues are escalated.
Liaise with booking teams to be a single point of liaison for referrals, booking and coordination of patient
Coordinator and plan the screening review meeting to ensure that key performance targets are achieved.
Communicate information on the patient's progress on the care pathway to LCS Nursing Team Leader
Communicate regularly with patients to ensure that they are aware of upcoming appointments, diagnostic investigations and offer practical support with the arrangements. Being sensitive to the fact that patients may be anxious and distressed.
Provide basic clinical information to patients in line with the agreed pathway and learn to recognise urgent results and act upon effectively.
Develop positive working relationships with all members of the team, liaising in particular with management, consultants, specialist nurse and other administrative staff

Essential
Essential
- o5 GCSE's including Maths and English at grade C or equivalent
- Knowledge of Microsoft Office packages e.g. Word, PowerPoint, Excel and other IT skills acquired through training and practical experience including internet/web skills
- Knowledge of medical terminology
- Relevant Health or Social Care experience
- NVQ Level 3 or equivalent experience
- Experience working with the public / patients in a healthcare environment
Desirable
- Level 3 Health or Social Care qualification
- Experience coordinating a patient workload
- Knowledge and experience of using Trust IT/patient record/information systems, e.g. ICS, ICE, WebV, Winscribe, CRIS
- Experience of line managing staff and dealing with issues that can arise with staff management
