Job description

  • Communicate with relevant departments and organisations to ensure that the patient's pathway is as timely as possible and all delays are avoided.
  • Communicate with key departments (diagnostics, clinic bookings etc.) to ensure all tests and investigations are booked promptly and any issues are escalated.
  • Be a single point of contact for patients, carers and the healthcare team - triaging sensitive enquiries and referring to the appropriate department within the hospital. Using own judgement as to what requires immediate action.
  • Communicate regularly with patients to ensure that they are aware of upcoming appointments, diagnostic tests/investigations and operations and offer practical support with the arrangements. Being sensitive to the fact that patients may be anxious and distressed.
  • Provide support and contribute to training of new / junior members to the team.
  • Document and monitor all aspects of patient care coordination and service delivery onto the Trust IT systems e.g. Cancer Access Database, Winscribe, WebV, ICE, ICS, CRIS,PPM database etc. to ensure the patient's progress can be measured.
  • Ensure that electronic notes are correctly maintained following guidelines and that the documentation contained therein is correct. High quality standards are to be always maintained.

Experience

Essential

  • Previous administrative and organisational experience
  • Relevant Health or Social Care experience
  • Minimum of two years' experience working with the public / patients in a healthcare environment

Desirable

  • Experience of working in a hospital environment
  • Knowledge and experience of using Trust IT/patient record/information systems, e.g. ICS, ICE, WebV, Winscribe, CRIS
  • Previous experience working within a cancer service
