Lead Practice Nurse

• Provide day-to-day clinical leadership, guidance and professional support to the practice nursing team.
• Deliver a full range of Practice Nurse duties appropriate to the Lead Practice Nurse role.
• Lead on chronic disease management including asthma/COPD, hypertension, diabetes and cardiovascular disease.
• Support achievement of QOF and other relevant contractual and quality requirements.
• Lead and coordinate vaccination and immunisation programmes, including seasonal and national campaigns.
• Act as clinical lead for infection prevention and control.
• Undertake and oversee treatment room services including cervical screening, immunisations, wound care and ECGs.
• Review and develop clinical protocols, procedures and systems.
• Lead and support clinical audit, service evaluation and quality improvement.
• Oversee clinical supplies, equipment, vaccine stocks and cold-chain requirements.
• Work closely with the Practice Manager and GP Partners on nursing workforce, capacity and service development.
• Maintain high clinical standards, accurate SystmOne records and compliance with NMC requirements.
• Work collaboratively with the wider practice, PCN and community healthcare teams.

• Registered Nurse with current NMC registration.
• Recognised Practice Nursing qualification or demonstrable equivalent experience and competencies within General Practice.
• Current cervical screening qualification and competency.
• Current immunisation and vaccination training and competency.
• Recognised training and competency in chronic disease management.
• Evidence of ongoing continuing professional development relevant to General Practice nursing.

Desirable

• Independent/Supplementary Prescribing qualification.
• Leadership or management qualification.
• ARTP qualification or equivalent recognised respiratory training.
• Mentorship, clinical supervision or teaching qualification.
• Additional qualifications in long-term condition management.

• Significant and recent experience of working as a Practice Nurse within General Practice.
• Extensive experience in chronic disease management, including asthma/COPD, hypertension, diabetes and cardiovascular disease.
• Experience of delivering structured long-term condition reviews and recall systems, including LTC reviews by birth month.
• Experience of cervical screening.
• Extensive experience of childhood and adult immunisations and vaccination programmes.
• Experience of working autonomously and managing an independent clinical caseload.
• Experience of providing leadership, supervision and clinical support to members of a nursing team.
• Experience of QOF and achieving relevant clinical targets.
• Experience of infection prevention and control within General Practice.
• Experience of clinical audit, quality improvement and implementation of clinical protocols.
• Experience of multidisciplinary working within primary care.
• Experience of managing or coordinating seasonal and national vaccination programmes.

Desirable

• Experience of using Ardens templates, searches and clinical resources to support long-term condition management, QOF and standardised clinical recording.
• Previous experience in a Lead Practice Nurse or Senior Practice Nurse role.
• Experience of staff supervision, appraisal and competency assessment.
• Experience of managing nursing workload, capacity and skill mix.
• Experience of developing and reviewing clinical policies and protocols.
