Shore Medical is seeking an enthusiastic and compassionate Integrated Care Team GP to support our housebound and care-home population. The post holder...
- Provide comprehensive, person-centred GP care to housebound and care-home patients with frailty, multimorbidity and complex health needs.
- Undertake proactive assessment, diagnosis, treatment, medication reviews and personalised care planning.
- Work collaboratively with the multidisciplinary team, care homes, patients, families and carers to coordinate care and support safe transitions between services.
- Identify patients at risk of deterioration, support anticipatory and end-of-life care, and reduce avoidable hospital admissions.
- Contribute to population health, preventative care, quality improvement and proactive management of the PCN's housebound and care-home population.
- Provide comprehensive, person-centred GP care to a defined cohort of housebound and care-home patients.
- Hold a weighted lost of approximately 130 patients per session, with appropriate prioritisation according to clinical need and complexity.
- Undertake proactive reviews of patients with frailty, multimorbidity and complex health needs.
- Provide face-to-face and remote clinical consultations as clinically appropriate.
- Assess, diagnose and manage acute and chronic medical conditions.

Experience
Essential: - Worked within a UK GP Practice; - Working in Primary Care in an area with significant health problems; - EPS prescribing
Qualifications
Essential: - GMC Registered; - MBBS or equivalent medical degree; - MRCGP; - Inclusion on the Performers List; - MPS/MDUS/MDDUS membership; - Enhanced DBS Check
