The post holder is identified as a Hospital at Home Team Nurse, working in conjunction with the current H@H teams, providing skilled nursing services ...

1. To be responsible for the assessment of care needs, delivery and implementation of management plans for individual patients using a problem solving approach and evidence based care. 2. To work with colleagues, or independently, to provide the highest possible standard of care for patients assessed as safe and medically suitable to remain at home3. To work with specific groups of patients within agreed pathways in Primary and Secondary Care, and taking responsibility for their ongoing management and safety.4. To effectively assess identified or referred patients on to the Navigation or Hospital at Home Service as part of the admission avoidance scheme, and act on findings as per protocols. 5. To participate in effective and safe discharge planning of all identified inpatients/ outpatients and to promote smooth transition to the Hospital at Home Team or other community based services and teams. 6. To support patients in their home environment as part of Hospital at Home Virtual Ward, ensuring safe and uncomplicated recovery. 7. To develop effective working relationships with the all relevant community services enabling good integrated care with seamless handover as appropriate. 8. To receive and act on referrals direct from all areas within the Acute and Emergency Settings, for assessment, agreed tests and management of patients' condition. 9. Develop competence of professional practice to advice and support patients, using appropriate protocols.

Experience

  • Experience working as Band 6 within an acute specialism or Band 5 with the relevant nursing experience
  • Some managerial experience
  • Evidence working in acute setting
  • Experience of supervising junior staff

Knowledge

Essential

  • Understanding of Clinical Governance
  • Knowledge of risk management
  • Audit and research and evidence based practice
  • Knowledge of National and Local Policy agenda's relating to post

Knowledge

Desirable

  • Clinical experience of working with community services
  • Training of history taking and knowledge of coagulation
