To provide a clinical service that is patient focussed, accessible and responsive on a 7-day a week basis.
To triage all mental health referrals into the Team to determine the priority for service response and the appropriate clinical pathway.
To provide and deliver highly skilled nursing care, including full mental health assessment, care planning and care delivery, biopsychosocial interventions, clinical risk assessment and management, and promoting recovery and inclusion.
To recommend and refer, where treatment is indicated, to the most appropriate team/service.
To work collaboratively with the crisis house to lead on care for admissions as a service.
To facilitate early discharge from the inpatient wards where clinically indicated.
To deliver home based treatment interventions for all patients, ensuring best practice is implemented.

Qualifications
Essential
- Registered Mental Health Nurse and current professional registration with the NMC or registered allied health professional.
- Key skills in literacy, numeracy and IT Level 2 or equivalent.
Desirable
- Evidence of Continuing Professional Development.
- Risk Assessment and Management Training.
- Suicide Prevention Training.
- Supervision Training.
