As a Therapy Assistant within the Home First service, you will support people in their own homes following hospital discharge to maximise independence, recovery and wellbeing. Working as part of a multidisciplinary team alongside Occupational Therapists, Physiotherapists and other health and social care professionals, you will manage a delegated caseload and deliver person-centred therapy and reablement interventions within your competency framework.
You will undertake holistic assessments, monitor progress, and adapt interventions to support individuals to achieve their goals and maintain independence at home. The role includes supporting mobility, daily living activities, functional rehabilitation, risk assessment, and the provision of equipment and minor adaptations where appropriate.
You will provide advice and education to individuals, families and carers, maintain accurate clinical records, contribute to discharge planning, and work closely with colleagues and partner organisations to ensure coordinated care.
The post holder will participate in supervision, training and continuous professional development, contribute to service improvement initiatives, and support the development of less experienced staff, students and learners where appropriate.

Qualifications
Essential
- Relevant Level 3/4 qualification in Health & Social Care or a related healthcare subject.
- GCSE (Grade 4/C or above) or equivalent in English and Maths.
Desirable
- Level 5 qualification/apprenticeship
- Care Certificate
Experience
Essential
- Experience working in health, social care or community services.
- Experience working within a therapy or multidisciplinary team.
- Experience promoting independence and supporting rehabilitation.
- Experience educating or supporting patients, carers or colleagues.
- Strong communication, organisational and IT skills.
- Ability to maintain accurate clinical records and manage risk appropriately.
- Ability to work flexibly across a 7 day service
- Full driving licence/access to a vehicle
Desirable
- Experience managing a delegated caseload and working independently.
- Experience of working with people with long term conditions
