An exciting opportunity has arisen for an experienced and motivated Heart Failure Specialist Nurse to join the Coventry and Rugby Heart Failure Team. ...
You will review patients through face-to-face and virtual clinics, Hospital at Home, home visits and telephone consultations.
You will work closely with the multidisciplinary team (MDT), GPs, healthcare professionals, patients and carers to ensure safe, effective and personalised care.
- Provide specialist assessment and management of patients with heart failure.
- Work autonomously across acute, community and virtual care settings.
- Lead and support the Heart Failure Nursing Team.
- Develop and implement evidence-based treatment plans.
- Support optimisation of heart failure medication in line with current guidelines.
- Refer patients for cardiac rehabilitation where appropriate.
- Consider and refer patients for cardiac device therapy when indicated.
- Promote patient education, self-management and improved outcomes.
- Liaise effectively with the MDT, GPs, healthcare professionals, patients and carers.
- Promote best practice in heart failure management in accordance with national and local guidelines.

Qualifications
Essential: - oNMC Registration oHeart failure Module level 6/7 or equivalent oAdvanced Clinical Assessment Skills Level 6/7. oStudy at level six (degree/diploma)
Desirable: - oNon - Medical prescribing Level 6/7 or willing to undertake oAdvanced communication Course
Experience
Essential
- Extensive experience in heart failure
- Optimising HF medicines, referrals for cardiac devices and other therapies.
- Experience in HF Management
- Manageing patient on H@H
Desirable: - oLeadership course oAbility to run Nurse led Clinics.
Knowledge
Essential
- Extensive knowledge of heart failure and management
- Ability to present healthy lifestyles information to both patients as individuals and in groups.
- Extensive knowledge and understanding of NICE 2025 and ESC 2026 guidance on HF management
Desirable: - oCardiac rehabilitation
