To provide individualised palliative care to patients as part of the Hospice at Home night service, either through the provision of one-to-one night s...
To work with, and support, the patient, relatives, and carers; being sensitive to their changing physical, psychosocial and spiritual needs.
To provide skilled nursing care within the plan of care, and in accordance with the medical care prescribed.
To help/prompt the patient to self-medicate if it is part of their nursing care plan or to assist patient with medication administration once Level 1 Support for the Patient with Prescribed Medication course is completed.
Demonstrate simple nursing procedures, where appropriate, to carers or colleagues to assist in promoting and maintaining patient comfort.
To communicate as required with the Roaming Service, out of hours services, on-call nurse and/or Hospice at Hospice at Home coordination team.
Advise the Hospice at Home Coordination team of equipment and aids required for the provision of efficient nursing care to meet the needs of the patient.
To be familiar with the procedure in the event of an emergency or death of the patient.
To maintain professional confidentiality in all matters relating to patient care.
To follow the instructions given for the safer patient handling, and the use of handling equipment.
To keep up to date with developments in nursing care and treatments.
To report any complaints immediately as per policy
To be accountable to Treetops Hospice Care in line with the Code of Behaviour as documented in the Hospice at Home staff handbook.
To provide a

life expectancy, to enable them to remain at home if that is their wish. It is
intended to be complementary to existing community services.

co-operation with the Primary Health Care Team and other care agencies to
provide support and skilled nursing care to those in the terminal phase of
their illness, or with chronic incurable conditions.

Qualifications
Essential
- National Vocational Qualification (NVQ) level 2 in Health & Social Care
- Demonstrable clinical competence in palliative care or a
- willingness to undertake additional training.
- Willing to travel to attend meetings or undertake training if required.
- Take responsibility for own development.
- Willingness to cover day services to maintain competencies and cover service needs.
- Experience of delivering palliative care
- Excellent communication and interpersonal skills
- Good record keeping
- Willing to attend clinical supervision to reflect on practice.
- IT literate
- Understands the importance of confidentiality.
- Basic needs assessment skills
Desirable
- NVQ Level 3 Health and Social Care or equivalent
- Palliative and/or end of life care training.
- Dementia training or experience
- Tier 1 or 2 Communications skills training
- Community care experience
- Knowledge/experience of system one
- Understanding risk management
- Knowledge and understanding of current end of life care pathways and Palliative symptoms.
- Experience in bereavement support
- Knowledge of inter-disciplinary working
- Experience of caring for patients with a range of life limiting illnesses.
- Proactive in supporting colleagues
