The Healthcare Assistant (HCA) will work as an active member of the team to provide a high quality, efficient and effective service to patients who require visits at home.

The post holder will need to be able to drive and have access to a vehicle as travel to and from Patients' homes. Daily, mileage is claimable via the trust expenses system.

You may be asked to complete Patient visits in other teams depending on what is required on the day.

  • To deliver a high standard of patient care as a healthcare assistant in the community and report any significant outcomes or changes in conditions to senior team members.
  • To provide support within the team and work as a team player.
  • Responding effectively to patient needs and ensuring patient choice and ease of access to services.
  • To work within a competency framework appropriate to the individuals scope of practice.
  • To undertake clinical procedures, as delegated by senior staff, such as wound care, pressure ulcer care and management, equipment ordering, leg ulcer care, catheter changes and on-going care.
  • To advise carers and/or patients how to manage care in between visits and promote health education on an individual basis as directed by senior staff.

Qualifications

Essential

  • GCSE maths and English (or equivalent)
  • Care Certificate
  • NVQ Level 3

Desirable

  • NVQ Level 2

Skills & Knowledge

Essential

  • Previous experience in care/HCA role
  • Work safely with minimal supervision
  • Good interpersonal and communication skills both verbal and written
  • Prioritisation skills
  • Full UK Driving Licence
  • Access to a car

Desirable

  • Knowledge of wound care/catheter care/venepuncture

Experience

Essential

  • Previous experience in care - e.g. as a HCA or a Carer
  • Safe moving and handling experience

Desirable

  • Previous community experience
  • Previous NHS experience (ward based)
