Health and Wellbeing Coach - West Somerset Primary Care Network

Working across Exmoor National Park and the wider West

Somerset community, you will help people to connect or reconnect with nature

and their local communities in ways that support physical health, mental

wellbeing, social connection, confidence and independence. You will work

creatively with existing opportunities and partners, while developing new

nature-based approaches that respond to the needs and interests of local

people.

You will work with a diverse range of people and

communities, including those who may experience barriers to good health, social

isolation or reduced confidence. You will build trusting, compassionate and

non-judgemental relationships, recognising the strengths and potential within

each person and supporting them to become active participants in their own

health and wellbeing.

As a member of the West Somerset Primary Care Network

multidisciplinary team, you will work alongside GPs, Social Prescribing Link

Workers, Care Coordinators, Health and Wellbeing Coaches and community

organisations, while developing close working relationships with colleagues

across Exmoor National Park Authority, Somerset Council and the voluntary,

community and social enterprise sector.

The successful candidate will be compassionate,

reflective and self-aware, with a genuine passion for supporting people to

improve their health and wellbeing through personalised care and meaningful

connections with nature. They will enjoy working with people from diverse

backgrounds and recognise the strengths, skills and potential that exist within

every individual and community.

They will possess excellent communication, coaching and

relationship-building skills, with the ability to inspire confidence, motivate

others and work collaboratively across organisational boundaries. They will

provide a high-quality, non-judgemental and person-centred service, enabling

individuals to identify what matters most to them and supporting them to

achieve their own health and wellbeing goals.

They will be proactive, innovative and adaptable, with

the ability to develop partnerships that bring together healthcare, local

government, Exmoor National Park, voluntary organisations and local communities

to deliver sustainable, place-based solutions for health and wellbeing. They

will be committed to continuous learning, reflective practice and contributing

to the ongoing development and evaluation of Healthy Exmoor as an innovative

model of Green Social Prescribing and personalised care.

You will deliver the Healthy Exmoor programme through a personalised care approach, supporting individuals to improve their physical, mental and social wellbeing through health coaching and meaningful engagement with nature.

You will also manage and prioritise a diverse caseload, providing person-centered support that empowers individuals to identify what matters to them, build confidence, increase self-management and improve their health and wellbeing outcomes.

You will develop and facilitate nature-based health

and wellbeing opportunities that utilise Exmoor National Park and the wider

natural environment of West Somerset as assets for improving health, wellbeing

and reducing health inequalities.

Healthcare Services (Symphony) provides NHS primary care provision across 21

sites within Somerset, linked to 14 general practice contracts. The

organisation cares for approximately 132,000 patients and employs around 600 staff.

Symphony is a subsidiary of

Somerset NHS Foundation Trust and manages its services through the support of a

central team (including HR, finance, corporate and operational governance).

Qualifications

Essential: - Willingness to complete a Personalised Care Institute (PCI) accredited Health and Wellbeing Coach training programme if not already qualified.
